The subject of the contract is the supply of a system for measuring blood coagulation along with the lease of the device for SPZOZ WSS No. 3 in Rybnik. The specification of deliveries and the subject of the lease are included in Appendix 1.1, i.e. "Price Form". Other terms and conditions of the contract are set out in Appendix 2 to the terms of reference ("Draft provisions of the contract", on public procurement to be incorporated into the contract). The agreement will be concluded after the end of these proceedings for a period of 36 months.
