The subject of the order supply system for research in the field of hematology with the lease of equipment for the Laboratory Diagnostics Department of Self-Public Health Care Department of the Provincial Specialist Hospital No. 3 in Rybnik. The specification of the order includes Annex 1 to this Specification of the Terms of Order.

The set of annexes constituting the characteristics of the subject of the contract and the basis for the calculation of the offer price shall consist of the following components:

1.1. required types of tests with an estimated quantity for a period of 48 months:

1.1.A. Price form of reagents and consumable materials for hematological tests;

1.1.B. Cumulative price form.

1.2. characteristics of leased devices. Hematological testing system with lease of devices — including analyzer: 2 pieces (primary and support analyzer).
