1. The subject of the contract is to accelerate the processes of digital transformation through the development of digital services for the District Health Center Ltd. in Kartuzy, by supplying equipment and software in the assortment, estimated quantities, and requirements specified in Annex No. 2 to the Terms of Reference (SWZ).

2. The subject of the contract is funded by the Instrument for Reconstruction and Resilience, implemented as part of the investment D1.1.2 "Accelerating the processes of digital transformation of health care through further development of digital services in health care," which is part of component D "Efficiency, accessibility, and quality of the health care system".

3. A detailed description of the subject of the contract, including quantities for individual Parts, is included in Annex No. 2 to the SWZ – in the assortment-price form along with the required parameters.

4. The Contractor undertakes as part of the contract to:

1) train the Ordering Party's employees in the use of supplied equipment or software - concerning Part No. 1,3,5,6,7,12

2) maintain manufacturer's technical support for the offered product for a period of 36 months from the date of signing the handover protocol – concerning Part No. 1,2,5,6,10,13

3) oversee the system of the offered product for a period of 36 months from the date of signing the handover protocol – concerning Part No. 3,6,9,12,13.

5. The products offered by the Contractor specified in Parts No. 1,2,3,6,10 must be:

1) brand new, complete, operational, and approved for circulation and use in the territory of the Republic of Poland,

2) free from any physical and legal defects,

3) not encumbered by the rights of third parties or claims in favor of the State Treasury in connection with import to the Polish customs area,

4) not previously exhibited or presented, non-refurbished, and non-reconditioned,

5) free of any type of locks that would make it difficult for the Ordering Party's employees to access the equipment after the warranty period,

6) comply with the requirements of applicable norms and legal regulations, as well as security and quality requirements specified by applicable legal regulations.

Part No. 1
Delivery of WI-FI controllers along with access points and installation.
The subject of the contract includes the delivery of WI-FI controllers along with access points and installation - ICT equipment for strengthening the wireless network infrastructure:

• controllers (replacement of outdated 1 unit and additional supply of 1 unit – HA mode),
• access points,
• other elements allowing for increased network capacity,
• licenses,
• implementation and configuration,
• training and maintaining technical support.

Part No. 2
Delivery of database servers and storage space server.
The subject of the contract includes the delivery of database servers and a server for storage space:

• delivery of ICT equipment - server (1 unit) with large storage space, with implementation and maintenance of technical support (enabling installation and effective functioning of IT systems in the hospital, ensuring stability, scalability, and secure storage of medical data),
• delivery of ICT equipment - 2 units of database servers with maintenance of technical support (equipped with ultra-fast NVMe drives, large RAM, and efficient processors, ensuring quick access to data, increasing efficiency, scalability, and reliability),

Part No. 3
Mobile devices, scanners.
The subject of the contract includes extending the existing system for document digitization (IC PEN):

• delivery of ICT equipment - 30 units of mobile devices with required licenses and maintenance service (complementary operation to the HIS development (workstations, enabling the creation of EDM and reviewing data at the patient's bedside, allowing for the collection of digital signatures from patients),
• delivery of ICT equipment - 8 units of scanners for digitizing medical documentation with licenses and maintenance service (enabling digitization of medical docs, including information cards from hospital treatment. The scanner parameters include automatic document feeders, fast double-sided scanning, integration with HIS. 7 scanners will be connected directly to the workstation, 1 network scanner will allow usage for several workstations),

Part No. 4
Delivery of database licenses along with migration service.
The subject of the contract includes the delivery of database licenses along with migration service:

• delivery of Software - 4 units of database licenses along with migration service and upgrade (replacement of 4 Oracle 11g licenses)

Part No. 5
Delivery of log analyzing software.
The subject of the contract includes the delivery of log analyzing software:

• Implementation of NAC-Network Access Control system (software) with implementation and training (allowing for effective network access management through identification, authentication, and control of devices and users, enforcing security policies, detecting unauthorized devices, supporting network segmentation and incident response, also increasing IT infrastructure security through real-time data analysis, necessary due to the development of EDM and HIS),
• delivery of Virtual Software for collecting and analyzing system logs with implementation, training, and maintenance service (System using ML to analyze large datasets for detecting anomalies and threats, based on AI solutions that support threat analysis and automation of responses, enabling learning from past incidents, improving detection and response effectiveness)

Part No. 6
Delivery of devices along with backup system.
The subject of the contract includes the delivery of devices along with the backup system implementation:

• delivery of ICT equipment - 2 units of backup devices with BC and DR function: (enabling backups, deduplication, and data recovery ensuring cybersecurity protection, zero-trust backup with protection against ransomware and with licenses, implementation, training, and maintenance service of the manufacturer: AIRGAP ensuring separation and off-site storage of offline backups, tests confirming accuracy and effectiveness of restoration)

Part No. 7
Extension of the currently held antivirus license by the Ordering Party.
The subject of the contract includes the implementation of EDR-Endpoint Detection and Response (software) with training: (ensuring protection of workstations, servers, and medical devices against cyber threats through continuous monitoring, behavior analysis, quick detection, automatic response to attacks and enabling detailed post-breach analysis)

Part No. 8
Access to the training platform.
The subject of the contract includes training services for managerial staff and medical and non-medical personnel (250 people) in raising awareness in the area of cybersecurity (cyber hygiene): (ensuring a better understanding of digital threats and appropriate behaviors, methods of recognizing and avoiding phishing attacks, ransomware, or data leaks).

Part No. 9
Delivery of applications for analyzing the number of Information Cards indexed in the P1 system.
The subject of the contract includes the delivery of Software for monitoring and analyzing the number of EDM indexed on the P1 platform: (enabling current validation of the accuracy and completeness of the transmitted medical data, which will improve reporting and identification of any integration errors, as well as allow effective monitoring of the requirement of 95% indexed information cards from hospital treatment and other types of EDM that should be indexed on the P1 platform according to regulations)

Delivery of edge routers along with configuration transfer from current devices.
The subject of the contract includes the delivery of Edge Routers (UTM) - 2 units with required licenses, configuration, settings migration, and manufacturer's technical support: (antivirus, web filtering, DNS filtering, IPS, and full SSL traffic decryption)

LOT-0011
Part No. 11
Implementation of the ISMS documentation, conducting penetration tests, conducting a final audit.
The subject of the contract includes the implementation of ISMS documentation, conducting penetration tests, conducting a final audit:

• Preparation/clarification of procedures required in the area of cybersecurity - a cryptography policy with consideration for recommended permissible encryption protocols, an event logging policy considering applications, networks, servers, edge gateways, domain control, and backup policy, compliant with the applicable ISO 27001 standard or equivalent,
• testing services of protective mechanisms necessary for data protection, preventing leaks, minimizing the risk of malware infection, and ensuring compliance with regulations. To verify the correctness and effectiveness of operation,
• final cybersecurity audit services allowing for an independent assessment of the security state of IT systems, identification of potential vulnerabilities, compliance with legal requirements, and increasing the organization's resilience to digital threats, etc.

LOT-0012
Part No. 12
Extension of the HIS system with additional functionalities.
The subject of the contract includes:

• Expansion of HIS (purchase of software) with implementation and maintenance during durability: expansion with EDM that CeZ is working on, integration with the external system, Physician's Desktop, Nursing, Anesthesiology, e-Anesthesiology Card, blood bank with serology, management of medication orders, monitoring of continuity of action; that will allow implementing new types of EDM and increasing the efficiency of medical staff work (e.g., desktops will allow access to data and creating EDM at the patient's bedside),
• Expansion of HIS (purchase of software) with implementation and maintenance during durability: integration with the e-Blood system and SIMP system, which will enable automatic filling of documents with data available in HIS, validation of their correctness, and sending directly from HIS,
• re-indexer type software with implementation and maintenance during durability enabling indexing in the P1 system of information cards from hospital treatment since 01.01.2023. Documents will comply with CeZ's requirements for EDM, including PIK HL7 CDA,
• Integration of the EDM repository with RIS with implementation and maintenance during durability of the project will enable automatic, compliant storage and sharing of imaging test results and descriptions, facilitating the flow of information and patient care. This integration is essential for indexing in the P1 system and the proper functioning of the integration with PUI, thereby benefiting from AI-based services on PUI,

Part No. 13
Extension of the RIS/PACS system with additional functionalities (integration with PUI).
The subject of the contract includes:

• Integration with PUI - interface for PUI with implementation and maintenance during durability ensuring the use of AI-based e-services supporting clinical decisions through preliminary descriptions of diagnostic imaging studies, which will facilitate quick and precise detection of pathologies in images.
