The subject of the contract are successive deliveries of medical devices used in ophthalmology for SPZOZ WSS No. 3 in Rybnik.
The delivery specification is contained in Annex 1.1 to the Terms of Contract ("Price Form"). Other terms of the contract are set out in Annex 2 to the Terms of Contract, i.e. "Draft provisions of the contract", which will be introduced into the content of the contract / contract provisions. The contracts will be concluded after the conclusion of these proceedings for a period of 24 months.
