DDD services are to be performed using the means and methods least harmful to people, the environment and in accordance with the currently applicable sanitary regulations. The subject of the contract is DISINFECTION, DISINFESTATION AND DERATIZATION SERVICE FOR ROOMS, PERSONAL EQUIPMENT AND POLICE SERVICE VEHICLES IN THE POMERANIAN GARRISON.

The offer should be accompanied by a completed calculation (Annex 1a to the SWZ) and a price offer.
The unit price should include the cost of the service, m.in. the value of materials used and the cost of travel to the indicated unit for:
a. 1 m2 of usable area (assuming a room height of 2.5-3.0 m),
b. 1 pc. additional equipment (m.in. blanket, mattress, pillow)
c. 1 pc. company vehicle,
d. 1 point of the lined preparation;
The service provider should have the necessary knowledge and experience as well as technical potential – the statement should be submitted when signing the contract.
The Contractor undertakes to perform the service ordered by phone within 24 hours from the moment of notification in the field of disinfection and disinfestation, and 48 hours in the field of deratization (this period includes travel time to the place of service).
The Contractor shall use disinfectants with a spectrum of action against viruses (e.g. HIV, HCV), fungi, bacteria and mycobacteria - guaranteeing proper performance of the service. Agents used for disinfection and disinfestation have certificates and permits of the Ministry of Health and Health Care and PZH proving that they meet current requirements and are approved for use in places where people are present and are respected by the EU The duration of action of the disinfectant must not exceed 60 minutes.
The Contractor is obliged to submit a statement that all chemical agents that will be used on the premises of the Contracting Authority have the certificates and permits required by law and are allowed for use in facilities where people are staying (in accordance with Annex 11 to the SWZ).
DDD services are to be performed using the means and methods least harmful to people, the environment and in accordance with the currently applicable sanitary regulations.
The Contracting Authority shall submit to the Contractor each time an order signed for the performance of the service, in which the exact scope of work will be specified, specifying – the number and area of rooms – confirmed by the head of a given unit.
The Contractor is obliged to make an appropriate entry in the "SANITARY CONTROL BOOK" located on the premises of a given facility, with a detailed description of the service provided.
The Contractor is obliged to issue a "Protocol" each time of the activities performed related to the subject of the contract as well as the means used and attach the document to the invoice. The "Protocol" shall be prepared by the Contractor in 2 copies, of which one copy shall be left in the ordering unit and the other copy shall be sent to the Contracting Authority together with the invoice.
In the event of non-performance or improper performance of the terms of the contract by the Contractor, the Contracting Authority shall commission its implementation to another entity, and shall charge the costs to the Contractor;
The Contractor shall issue an invoice to the payer:

and sent to the address - in relation to the facilities of individual units - respectively through the Municipal Headquarters / District Police Headquarters, to which they are organizationally affiliated.

All requirements set out in the documents indicated above constitute minimum requirements and their fulfilment is obligatory. Failure to comply with the minimum requirements will result in the rejection of the offer as incompatible with the terms of the contract pursuant to Article 226 (1) (5) of the Law Pzp.
