The subject of the order is the delivery and assembly of a single-module, multi-functional, remote-controlled telecom RTG diagnostic kit, together with integration with the PACS / RIS system owned by the customer, as well as delivery and installation of the PACS / RIS server with its integration with the hospital system, for SPZOZ in Hrubieszów.

1. The subject of the contract is the delivery and assembly of a single-module, multi-functional, remote-controlled telecom RTG diagnostic kit, together with integration with the PACS / RIS system owned by the client, as well as the delivery and installation of the PACS / RIS server with its integration with the hospital system, for SPZOZ in Hrubieszów .

2. The material scope of the contract includes in particular:

1) delivery and assembly of a single-module, multi-functional, remote-controlled telecom-grade RTG diagnostic kit, manufactured not earlier than in 2019

2) delivery and installation of a PACS / RIS server with a matrix capacity ensuring at least 5 years of operation and data archiving, including its integration with the hospital IT system,

3) integration of the set referred to in item 1) with the hospital's PACS / RIS system,

4) disassembly and utilization of your x-ray apparatus,

5) adaptation of the rooms indicated by the contracting authority to install the RTG set, including:

a) replacement of the floor covering along with the preparation of the ground,

b) replacement of lead doors and lead windows,

c) renovation of walls and ceiling in the studio (filling, priming, finishing coat, painting),

6) training on the use of the supplied RTG kit for min. 3 people indicated by the contracting authority,

7) training of an employee indicated by the contracting authority in the scope of inspection and simple repairs of the device, confirmed by issuing a personal certificate,

8) conducting acceptance tests in accordance with the Regulation of the Minister of Health of 18 February 2011 on the conditions for the safe use of ionizing radiation for all types of medical exposures (Journal of Laws of 2017, item 884)

9) obtaining the necessary permits for the use of the subject of the contract by the SPZOZ in Hrubieszów, including the authorization of the competent State Provincial Sanitary Inspector to use the subject of the contract,

10) providing technical documentation, in particular operating instructions in Polish (in paper and electronic version) and warranty documents,

11) other elements included in Annex 1 to the ToR and the Draft Contract constituting Annex 2 to the ToR

3. A detailed description of the subject of the contract, including the technical parameters of the required equipment, is included in Annex 1 to the ToR

4. The supplied equipment must be brand new. The Employer does not allow exhibition equipment, reconditioned etc.

5. The apparatus offered must be complete and ready for use without additional purchases.

6. The Purchaser requires a guarantee for the supplied X-ray apparatus for a period of not less than 24 months from the date of receipt of the subject of the contract. The Employer has full rights under the warranty for physical defects arising from the provisions of the Civil Code within the time limits specified therein - regardless of the rights under the warranty.

7. The Employer requires the Contractor to have civil liability insurance for conducting business activity to the extent consistent with the subject of the contract for an amount equivalent to the remuneration referred to in § 9 para. 2 of the draft contract, valid for the entire duration of the contract. Detailed conditions are specified in § 5 of the Draft Contract.
