A detailed description of the subject of the contract, a description of the ordering party's requirements for implementation and acceptance are specified by: - description of technical parameters - Annex 2 to SWZ - package 1 (computer tomograph) and Annex No. 2a to SWZ - package No. 2 (X-ray apparatus) - warranty and post-warranty service conditions - Annex No. 7 to SWZ - package 1 (computer tomograph) and Annex No. 7a to SWZ - package No. 2 (X-ray machine). - draft provisions of the agreement – Annex 4 to the SWZ.

The execution of the order includes: a) delivery and assembly of the apparatus. b) conducting instructions of the Ordering Party's medical staff in the scope of its operation and rational operation, c) providing a user manual in Polish for a computer tomograph and all additional equipment, d) providing a guarantee for the delivered devices in accordance with the conditions contained in Annex 7 to the SWZ – package 1 (computer tomograph) – warranty and post-warranty service conditions. e) handing over the device for use on the basis of a delivery and acceptance protocol signed by both parties, which will be the basis for issuing a VAT invoice.
