Delivery of medical and non-medical supplies and medical equipment for the Copernicus Hospital

1. The subject of the contract is the supply of medical and non-medical articles and medical equipment for the Copernicus Hospital

2. The subject matter of the contract has been divided into seventeen parts.

3. A detailed description of the subject of the contract, specifying the requirements of the Contracting Authority and quantities, is included in Appendix No. 1 to the Terms of Reference – Assortment and Price Form and in Appendix No. 5 to the Terms of Reference – Draft Agreement. By filling in the items indicated in Appendix No. 1 to the Terms of Reference, the Contractor confirms that the requirements specified by the Contracting Authority are met.

4. The offered objects of the contract must be complete, of a high standard both in terms of quality and functionality, as well as free from defects in material, construction and law.

5. The offered items of the order must be admitted to trading and use in accordance with applicable law.

Surface cleaners

1. The subject of the contract is the supply of surface cleaning preparations.

2. Item name according to the Common Procurement Vocabulary: 39830000 - cleaning agents.

3. A detailed description of the subject of the contract, specifying the requirements of the Contracting Authority and quantities, is included in Appendix No. 1 to the Terms of Reference – Assortment and Price Form and in Appendix No. 5 to the Terms of Reference – Draft Agreement. By filling in the items indicated in Appendix No. 1 to the Terms of Reference, the Contractor confirms that the requirements specified by the Contracting Authority are met.

4. The offered objects of the contract must be complete, of a high standard both in terms of quality and functionality, as well as free from defects in material, construction and law.

5. The offered items of the order must be admitted to trading and use in accordance with applicable law.

6. In all provisions of the SWZ and its annexes, in which the Ordering Party refers to standards, approvals, technical specifications or reference systems or trademarks, parameters or sources of origin (names of manufacturers or devices) are indicated, in accordance with Article 99(5) and Article 101(4) of the PPL Act, the Ordering Party shall allow equivalent solutions.

7. The subject of the contract must comply with the Act of 7 April 2022 on medical devices (i.e. Journal of Laws 2022.974 of 2022.05.09.), if the Ordering Party requires offering an assortment constituting a medical device.

8. The Contracting Authority shall not reserve the possibility of applying for a contract only by the Contractors referred to in Article 94 of the Pzp Act.

9. The Contracting Party shall not make the requirements referred to in Article 96 of the pzp Act.

10. In order to participate in the procurement procedure, a bid bond of PLN 2,750 must be paid. The tender deposit must be paid before the deadline for submission of tenders and maintained continuously until the expiry of the deadline for binding the tender

11. Source of financing: own funds.

Car washer disinfector

1. The subject of the contract is the supply of a disinfector washer.

2. Item name according to the Common Procurement Vocabulary: 33191000 - sterilization, disinfecting and hygienic equipment.

3. A detailed description of the subject of the contract, specifying the requirements of the Contracting Authority and quantities, is included in Appendix No. 1 to the Terms of Reference – Assortment and Price Form and in Appendix No. 5 to the Terms of Reference – Draft Agreement. By filling in the items indicated in Appendix No. 1 to the Terms of Reference, the Contractor confirms that the requirements specified by the Contracting Authority are met.

4. The offered objects of the contract must be complete, of a high standard both in terms of quality and functionality, as well as free from defects in material, construction and law.

5. The offered items of the order must be admitted to trading and use in accordance with applicable law.

6. 1. The subject of the contract is the delivery of a laminar flow cabinet.

1. The subject of the order is the supply of toothbrushes, drinkers, trays, guillotines, mortars, jars, glasses.

2. Item name according to the Common Procurement Vocabulary: 33190000 - various medical devices and products.

3. A detailed description of the subject of the contract, specifying the requirements of the Contracting Authority and quantities, is included in Appendix No. 1 to the Terms of Reference – Assortment and Price Form and in Appendix No. 5 to the Terms of Reference – Draft Agreement. By filling in the items indicated in Appendix No. 1 to the Terms of Reference, the Contractor confirms that the requirements specified by the Contracting Authority are met.

4. The offered objects of the contract must be complete, of a high standard both in terms of quality and functionality, as well as free from defects in material, construction and law.

5. The offered items of the order must be admitted to trading and use in accordance with applicable law.

6. In all provisions of the SWZ and its annexes, in which the Ordering Party refers to standards, approvals, technical specifications or reference systems or trademarks, parameters or sources of origin (names of manufacturers or devices) are indicated, in accordance with Article 99(5) and Article 101(4) of the PPL Act, the Ordering Party shall allow equivalent solutions.

7. The subject of the contract must comply with the Act of 7 April 2022 on medical devices (i.e. Journal of Laws 2022.974 of 2022.05.09.), if the Ordering Party requires offering an assortment constituting a medical device.

8. The Contracting Authority shall not reserve the possibility of applying for a contract only by the Contractors referred to in Article 94 of the Pzp Act.

9. The Contracting Party shall not make the requirements referred to in Article 96 of the pzp Act.
