Description:
Waste transport and loading service

1. The subject of the contract is the service of transport and loading of waste.

2. The procedure shall be divided into two parts:

Part I – Service of loading waste, sand and salt onto cars at the disposal of MPGK – Rzeszów Sp. z o.o.;

Part II – Waste transport service.

The service of loading waste, sand and salt on cars at the disposal of MPGK – Rzeszów Sp. z o.o.

1. The subject of the contract is the service of loading municipal waste and materials to combat road slipperiness onto cars at the disposal of the Ordering Party.

2.Us lang should be made with a wheel loader with the following parameters: bucket capacity minimum 5 m3, arm length minimum 5 m, lifting height minimum 6 m.

3.The scope of the order includes:

1) Loading of municipal waste in the Waste Handling Hall at 11 Ciepłownicza Street in Rzeszów, from Monday to Friday from 800 to 1800, as well as at other times

and days when it results from the legitimate needs of the Ordering Party.

2) Loading of materials to combat slipperiness in the salt and sand warehouse at each request of the Ordering Party.

3) Performing other works requiring the use of the charger in the place indicated by the Ordering Party at each request of the Ordering Party.

4) Performing duty in Rzeszów at 11 Ciepłownicza Street.

4. The Ordering Party reserves the right to unilaterally determine the time of hourly duty and change the time of commencement and end of the charger's work depending on the needs dictated by weather conditions.

1) the estimated number of hours of operation of the charger; up to 2300,

2) the estimated number of hours on-call time; up to 150.

5. The Contractor shall report to work at each telephone call of the Winter Action dispatcher immediately, but within a time not exceeding 1 hour (telephone number agreed with the Ordering Party).

6.The Contractor shall provide a backup charger in case of a possible failure of the primary loader.

7. The Ordering Party shall provide the Contractor with the possibility of using social facilities in the facility at 11 Ciepłownicza Street, to the agreed extent.

Part II – Waste transport service

1. The subject of the contract is the transport of waste by the Contractor with the following codes:

20 03 01 – non-segregated (mixed) municipal waste,20 02 03 – other non-biodegradable waste,19 12 10 – combustible waste (alternative fuel),19 12 12 – other waste (including mixed substances and objects) from the mechanical treatment of waste other than those mentioned in 191211,20 01 08 – biodegradable kitchen waste,20 02 01 – biodegradable waste (mowed grass, leaves, branches),20 03 03 – waste from cleaning streets and squares,17 01 01 – concrete waste and concrete rubble from demolitions and repairs,17 01 07 – mixed waste from concrete, brick rubble, waste ceramic materials and equipment other than those mentioned in 17 01 06.15 01 07 – glass packaging,20 03 07 – bulky waste,15 01 02 – plastic packaging,

loaded on the premises of the Waste Management Plant at 11 Ciepłownicza Street in Rzeszów to the place of waste management, in accordance with the current indications of the Ordering Party.

The subject of the contract includes the transport of waste with loading by the Contractor loaded on the premises of the Waste Management Plant at 11 Ciepłownicza Street.

The ordering party estimates that there will be about 36,000 Mg of waste to be transported, most of them will be

2. The Ordering Party does not guarantee the minimum amount of waste to be exported by the Contractor.

3. Waste should be transported to the recipient of waste designated by the Ordering Party and unloaded at the place indicated by the recipient. Loading of waste will take place during the working hours of the plant, i.e. from 6:00 to 17:00 and at any other time resulting from the needs of the plant after prior notification of such a need by the Ordering Party. Notification of the need to provide vehicles for loading waste will be made by phone, fax or e-mail at least 4 hours in advance.

4.Za laying of waste will take place on the basis of a collected transport order specifying the waste code, collected on the scale at 11 Ciepłownicza Street in Rzeszów.

5. The Ordering Party shall, as the waste transferor, issue a "waste transfer card", which will be confirmed by the Contractor each time.

6.The contractor, after loading the waste on his own vehicle, becomes their owner, which is consistent

with the Waste Act.

7. Waste transport will be carried out mainly in the period from Monday to Friday (occasionally on Saturdays), while the Contractor is obliged to ensure the disposal of waste in quantities resulting from the current needs of the Ordering Party.

8.The unit of account shall be tonne-kilometres. The distance from the place of waste collection to the place of its management will be calculated using the Googole Maps Internet Locator, available at www.maps.google.pl. The waste will be weighed twice, i.e. at the exit from the facility at Ciepłownicza Street and at the recipient of the waste, while the weight of the waste shown on the weight of the Ordering Party will be taken into account for the transport.

9. Waste transport will take place along the shortest route to the destination indicated by the Ordering Party. If the Contractor performs transport on a longer route, it does not acquire the right to remuneration in excess of the amount due for transport along the shortest route.

10. When performing waste transport, the Contractor should comply with all applicable laws and bears full responsibility in this respect, including responsibility for any cards and charges imposed on the Ordering Party in connection with the violation of road traffic regulations by vehicles transporting waste.

11. In the event of a breakdown of the basic vehicle intended for the transport of waste, the Contractor should substitute a replacement vehicle that met the
