The subject of the contract is a Service consisting in conducting training, instructional and educational and cultural activities supporting the introduction of new social functions in the Support and Social Development Center (CWiRS) at 34 Pogonowskiego Street in Łódź. . The order consists of 10 tasks, in accordance with the Detailed description of the subject of the contract, located in Annex 1 to the SWZ.

CONTRACT FOR SOCIAL AND OTHER SPECIFIC SERVICES at the award of which the provisions of the Act of 11 September 2019 apply.

- Public procurement law, applicable to classic contracts, with a value equal to or exceeding the EU thresholds - if the value of the contract expressed in PLN is equal to or exceeds the equivalent of EUR 750,000, taking into account the changes resulting from Section IV, Chapter 4 of the PPL.

1. The subject of the contract is a Service consisting in conducting training, instructional and educational and cultural activities supporting the introduction of new social functions in the Support and Social Development Center (CWiRS) at 34 Pogonowskiego Street in Łódź.

The order consists of 10 tasks, in accordance with the Detailed description of the subject of the contract, located in Annex 1 to the SWZ.

Task 1 - STAROPOLESKA SEPTEMBER

Task 2 - Summer with a Neighborhood Trainer;

Task 3 - Guided walking tours of the revitalization area;

Task 4 - Streetworking activities in the area of Project 1 ROCŁ and the immediate vicinity of CWiRS consisting in youth work;

Quest 5 - Wandering Civic Tent;

Exercise 6 - Workshops for parents and teachers ;

Task 7 - Training trip for young people with a series of workshops and elements of regional and civic education;

Task 8 - Duty hours of specialists;

Exercise 9 - Open Support Group for families and carers of people with mental illnesses and disorders;

Task 10 - Training for people working with people in homelessness crisis;

2.The purpose of the Order is the implementation of training, instructional and educational and cultural activities. The service concerns activities related to the introduction of new social functions at 34 Pogonowskiego Street. In the renovated property it is planned to launch: Community Self-Help House, Social and Professional Activation Center, Training Apartments with elements of social skills support, Lodz Multicultural Center, Lodz Civic Center, Consultation and Educational Point for Youth, Medical Consultation Point, premises for NGOs, creative studios, workshops for social activities, cafes.

3. The Contracting Authority does not specify the requirements for employment by the Contractor or subcontractor on the basis of an employment relationship of persons performing activities in the field of contract performance, because the performance of these activities does not consist, in the opinion of the Contracting Authority, in performing work in the manner specified in Article 22 § 1 of the Act of 26 June 1974 – Labour Code.

4. The Contracting Authority shall not require the Contractor to submit the said means of proof.

5.THE GROUNDS FOR EXCLUSION FROM THE PUBLIC PROCUREMENT PROCEDURE are described in point 7 SWZ.

6. Numbering in accordance with SWZ:

9.3.2 In order to demonstrate the absence of grounds for exclusion from post. The Contractor, the Contracting Authority demands:

9.3.2.1 Statements of the Contractor within the scope of Article 108 section 1 point 5 of the PPL, on the lack of belonging to the same capital group within the meaning of the Act of 16 February 2007 on competition and consumer protection with another Contractor who submitted a separate offer, or statements on belonging to the same capital group – model statement Appendix No. 10 to the SWZ.

9.3.2.2 Statements of the Contractor on the validity of the information contained in the NW. Statement:

- in Article 125(1) of the PPL, to the extent relating to the grounds for exclusion in Article 108(1)(3)-(6) of the PPL,

- Article 5k of Regulation 833/2014 as amended by Regulation 2022/576 and Article 7(1) of the Law on Support of Aggression against Ukraine and Serving the Protection of National Security. The model statement is attached as Annex 6a to the SWZ.

9.3.2.3. If the Contractor refers to the resources of other entities, the statement:

- Article 125(1) of the PPL in so far as it relates to the grounds for exclusion in Article 108(1)(3)-(6) of the PPL,

- Article 5k of Regulation 833/2014 as amended by Regulation 2022/576 and Article 7(1) of the Act on special arrangements for counteracting support for aggression against Ukraine and serving the protection of national security, is also submitted by each of these entities - model Annex 6b to the SWZ.

9.3.2.4 Information from the National Criminal Register regarding the grounds for exclusion specified in Article 108(1)(1), (2) and (4) of the PPL, drawn up no earlier than 6 months before its submission.
