The subject of the contract is the preparation of design and cost estimation documentation for the reconstruction of the road lane of Stryjeńskich Street on the section of Kazury 4 Street - Płaskowicka Street for the needs of communication services of the nursery at Kazury Street in Warsaw on The Journal of Laws No. 1; 2; 6; 29 from 01-11-02.
1) The subject of the contract is the preparation of design and cost estimation documentation for the reconstruction of the road lane of Stryjeńskich Street on the section of Kazury 4 Street - Płaskowicka Street for the needs of transport service of the nursery at Kazury Street in Warsaw on Journal of Laws No. 1; 2; 6; 29 from 01-11-02.
Stage Ia includes the preparation of multi-discipline project documentation:
1) road project of reconstruction of the road lane of Stryjeńskich Street in terms of parking, pavement and bicycle path along with land development;
2) a project of permanent traffic organization with the placement of signals (in connection with the opinion of the Road Traffic Management Office);
3) inventory of greenery,
4) geotechnical documentation,
5) drainage project
6) projects to remove possible collisions
7) bills of quantities
8) investor cost estimates,
9) Technical Specifications for the Execution and Acceptance of Works
10) Information on safety and health protection in accordance with Article 20(1)(1b) of the Construction Law.
Stage Ib includes obtaining on behalf of the Ordering Party a certificate of non-objection notification of the execution of works that do not require a building permit and submitting a paper version of the documentation in the form and quantity of copies in accordance with the Description of the Subject of the Contract.
Stage II – exercising author's supervision during the implementation of construction works, supplies and services.
The designer is obliged to exercise author's supervision over the implementation of works, supplies and services performed on the basis of design documentation, in accordance with the provisions of the concluded contract. The period of performing the author's supervision service will depend on the date of implementation of construction works carried out on the basis of design documentation. The Ordering Party shall notify the Designer of the commencement of the provision of the supervision service within the time limit set by the contract before its commencement.
A detailed description of the subject of the contract is specified in:
Description of the subject of the contract - Appendix No. 1 to swz;
the proposed provisions of the Agreement constituting Annex 5 to the SWZ;
2) SIMILAR ORDERS:
The Contracting Authority does not provide for the possibility of awarding contracts referred to in Article 214 (1) (7) of the Public Procurement Law Act.
3) SUBCONTRACTING
(a) The Contracting Authority shall not reserve the personal performance by the Contractor of key parts of the The Contractor may entrust execution of part of the contract to the subcontractor
(b) The Contractor is obliged to indicate in the offer the parts of the contract whose execution he intends to entrust to the subcontractors and provide the names of possible subcontractors if they are already known.
(c) Where the modification or resignation of a subcontractor relates to an entity whose resources the contractor relied on, under the rules laid down in Article 118 (1), in order to demonstrate that the conditions of participation in the proceedings are fulfilled, the contractor shall demonstrate to the contracting authority that another subcontractor proposed or the contractor independently satisfies them to a degree not less than the subcontractor whose resources the contractor relied upon during the procurement procedure. Article 122 of the Pzp Act shall apply accordingly.
