The subject of the order is the successive supply of antiseptics and disinfectants for the Self-Public Health Care Department of the Provincial Specialist Hospital No. 3 in Rybnik. The specification of deliveries shall be annexed 1.1 to this specification of the terms of the contract (“Price Form”).
