The subject of the order is the delivery of papers for Corpuls 3 and Lifepak 12/15 defibrillators, in detail specified in Annex 2 to SWZ — Description of the Object Order Price form in the quantity of 1 300 pcs. The quantities indicated in the price form are estimated/forecast values which may decrease during the course of the Agreement depending on the current needs of the Ordering Party. The Contracting Authority shall guarantee the use of a total of 60% of the value of the gross contract during the term

3.Communication between the contracting authority and the contractors is carried out in electronic form—point XII SWZ

4.The contracting authorities:does not provide for the conclusion of a framework agreement, does not allow and provide for the submission of variant tenders, does not establish a dynamic purchasing system, does not provide for the selection of the most favourable offer by electronic auction, does not provide for reimbursement of the costs of participation in the proceedings.

5. The offer shall be prepared in Polish on the Offering Form in accordance with Annex No 1 to the SWZ. Together with the offer, the Contractor is obliged to submit:

1) the statement in the form of the Single European Procurement Document (ESPD) referred to in Chapter IX (1) of the SWZ in accordance with Annex No 3 to the SWZ

2) the undertaking of another entity and the statement in the form of the Single European Procurement Document (ESPD) referred to in Chapter X (3) (1) and 2 of SWZ, in accordance with Annex No 4 to SWZ (if applicable);

3. Price form in accordance with Annex 2 to SWZ;

4) the documents from which the right to sign the tender arises; the relevant powers of attorney (if applicable).

6. Evidence measures in question - i.e. the sample referred to in Chapter IX (8) (1). Pursuant to Article 65 (1) (4) and paragraph 2 of Pzp, the Contracting Authority shall derogate from the requirement of electronic communication means and require the transfer of samples through a postal operator within the meaning of the Act of 23 November 2012 — Postal Law, personally or through a messenger. Samples shall be delivered to the Contracting Authority at the latest by the expiry of the deadline for submission of tenders indicated in Chapter XVII.

6.The Contractor shall submit an offer via a form to submit, modify, withdraw the offer or request available on ePUAP and also made available on the MiniPortal.
