The Contracting Authority divided the order in question into 5 parts including:

1) “Reception and management of municipal waste from inhabited properties located in Sector I in Kłobuck Municipality” — Part I”,

2) “Reception and management of municipal waste from inhabited properties located in Sector II in Kłobuck Municipality — Part II”,

3) “Reception and management of municipal waste from inhabited properties located in Sector III in Kłobuck Municipality — Part III,

4) “Reception and management of repairs and construction waste collected by owners of inhabited property, in big-bag bags directly from the resident's property site — Part IV”,

5) “Reception and management of the entire municipal waste stream from the Selective Waste Collection Point (PSZOK) located in Kłobuck at Łąkowa Street — Part V”

in accordance with separate Subject Descriptions of the Order which are annexes to the Specification of the Zam Terms.

1. The subject of the contract is the collection and management of the entire stream of municipal waste from properties located in Sector I within the Kłobuck Municipality in a way that ensures that adequate levels of recycling, preparation for reuse and recovery by other methods are achieved and limiting the weight of biodegradable municipal waste transferred to storage under the task assigned.

The basic data on Sector I as well as the obligations of the Contractor related to the execution of the contract are presented in the Description of the Subject of the Order for Part I, annex 4 to the Specification of the Order Terms and Conditions.

2. Number of multi-bar buildings located in Kłobuck Municipality: 34 (32 in Kłobuck, 1 in Łobodno, 1 in Kamyk).

3. Number of persons residing in property in Sector I according to declaration (as of 31.10.2021): 3062 persons.

4. The list of boxes and the quantity of containers of a specific capacity in a multilocal building from which the Contractor will be obliged to receive municipal waste is detailed in the Description of the Subject of the Order annex 4 to the Specification of the Conditions of Order.

5. The obligations of the Contractor are described in detail in the Description of the Subject of the Order annex 4 to the Specification of the Order Terms and Conditions.

1.The subject of the order is the collection and development of the entire stream of municipal waste from property located in Sector II in the Kłobuck Municipality:

- equipment of properties inhabited with containers for the collection of selectively collected glass and glass packaging,

- equipment of inhabited properties with bags for: plastics and plastic packaging, multi-material packaging and metal packaging; paper, cardboard and packaging made of paper and cardboard; waste that is biodegradable in a way that ensures that appropriate levels of recycling, preparation for reuse and recovery by other methods and reducing the weight of biodegradable municipal waste transferred to storage under the task assigned.

The basic data concerning Sector II and the obligations of the Contractor related to the execution of the contract are presented in the Description of the Subject of the Order for Part II, which is Annex 5 to the Specification of the Conditions of Order.

2. Number of inhabited detached property located in Sector II on the basis of declarations submitted: c. 2811.

3. Number of persons residing in Sector II property according to declaration (as of 31.10.2020): 7794.

4. Sector II includes single-family properties located in the following geodetic areas: Brody Malina, Kłobuck, Niwa — Skrzeszów, Osoki — Pustków, Przylatów, Rybno, Smugi, Zagórze, Zakrzew.

5. The scope of the contract also includes the collection of waste from the property with impeded access, due to the parameters and condition of the road surface specified in the table in paragraph 3.3 of the Description of the Subject of the Order annex 5 to the Specification of the Conditions of Order.

6. The obligations of the Contractor are detailed in the Description of the Subject of the Order, which is annex 5 to the Specification of the Order Terms and Conditions.

1. The subject of the contract is the collection and development of the entire stream of municipal waste from property located in Sector III in the Kłobuck Municipality:

- equipment of properties inhabited with containers for the collection of selectively collected glass and glass packaging,

- equipment of inhabited properties with bags for: plastics and plastic packaging, multi-material packaging and metal packaging; paper, cardboard and packaging made of paper and cardboard; waste that is biodegradable in a way that ensures that appropriate levels of recycling, preparation for reuse and recovery by other methods and reducing the weight of biodegradable municipal waste transferred to storage under the task assigned.

2. The basic data concerning Sector III and the obligations of the Contractor related to the execution of the contract are presented in the Description of the Subject of the Order for Part III, which is Annex 6 to the Specification of the Conditions of the Order.

4. Number of persons residing in property in Sector III by declaration (as of 31.10.2020): 6233

5. Sector III includes single-family properties located in the following geodetic areas: Biała Dolna, Biała Górna, Borowianka, Gruszewnia, Lgota, Libidza, Łobodno, Nowa Wieś, Mound, Pebble,

The scope of the contract also includes the collection of waste from the obstructed access property, due to the parameters and condition of the road surface specified in the table in paragraph 3.3 of the Description of the Subject of the Order, which is annex 6 to the Specification of the Conditions of Order.

7. The obligations of the Contractor are described in detail in the Description of the Subject of the Order, which is annex 6 to the Specification of the Order Terms and Conditions.

1. The subject of the order is the collection and management of repair and construction waste accumulated by the owners of inhabited property, in big-bag bags directly from the property of the resident.

2. The Contractor is obliged to cover the collection, transport and development service of selectively collected construction and demolition waste collected in big-bag bags received directly from the site of the resident's property. The amount of construction and demolition waste received directly from the property of residents of Kłobuck Municipality in big-bag bags from January to November 2021 was 75.72 Mg.

3. The obligations of the Contractor are described in detail in the Description of the Subject of the Order, which is annex 7 to the Specification of the Order Terms and Conditions.

1. The object of the order is the collection and development of the entire stream of municipal waste from the Selective Waste Collection Point (PSZOK) located in Kłobuck at Łąkowa Street.

2. The Contractor is obliged to provide a service for the collection and management of municipal waste received from the point of selective collection of municipal waste.

3. The Contractor is obliged to cover the collection, transport and management service of waste selectively collected the following waste:

- plastics (including packaging polystyrene), multi-material packaging, metals;

- paper and cardboard;

- glass and glass waste;

- textiles;

- spent batteries and batteries;

- used fluorescent lamps/fluorescent lamps;

- used electrical and electronic equipment;

- elements removed from worn devices;

- packages containing residues of hazardous substances;

- expired drugs and chemicals;

- furniture and other large-sized waste;

- construction and demolition waste, constituting municipal waste, which includes, inter alia: rubble, insulating materials (including roofing paper, polystyrene foam, mineral wool), sanitary ceramics, baths, shower cabins, building joinery;

- biodegradable waste;

- worn tyres from passenger vehicles and single-traced vehicles;

- ash from home hearth

4. The approximate mass of municipal waste selectively collected which should be received from PSZOK are listed in the table in point 3 of the Description of the Subject of the Order, annex 8 to the Specification of the Conditions of Order.

5. The obligations of the Contractor are described in detail in the Description of the Subject of the Order, which is annex 8 to the Specification of the Order Terms and Conditions.
