1. The subject of the contract is the provision by the Contractor to the Ordering Party of complex sanitary and hygiene procedures, transport activities, the performance of repair and maintenance work and cleaning services of external areas in the facilities of the Centre for Mental Health in Słupsk at 4 Defenders Coast Street in Słupsk.
2. The total area for cleaning is 5,423.33 m
1. The subject of the contract is the provision by the Contractor to the Ordering Party of complex sanitary and hygiene procedures, transport activities, the performance of repair and maintenance work and cleaning services of external areas in the facilities of the Centre for Mental Health in Słupsk at 4 Defenders Coast Street in Słupsk.
2. The total area for cleaning is 5,423.33 m
3. The list of rooms intended for cleaning together with the area shall be transmitted proto-collarally. Contractors are entitled to pay for the surface being cleaned up.
4. A detailed description of the subject matter of the order is set out in the following Annexes:
1. The list of areas intended for the performance of the service is set out in Annex 1 to IDW;
2. The range of operations and frequency of execution shall be determined by Annex No 2 to IDW;
3. The zones and scope of clean-up in each zone is defined by Annex 3 to IDW;
4. The working rules and the scope of the Contractor's responsibilities are set out in Annex 4 to IDW;
5. Contractor's responsibilities — repair and maintenance work is defined by Annex 4A to IDW;
6. Internal transport rules are defined by Annex 5 to IDW;
7. Whereas the requirements for washing and disinfectant preparations are set out in Annex 6 to IDW;
8. The hours of operation of the clean-up service is defined by Annex 7 to IDW;
9. The model control protocol is Annex 8 to IDW;
10. The model of the sanitary and hygiene control protocol is set out in Annex 9 to IDW;
11. The model of the thorough cleaning control protocol is set out in Annex No 9A to IDW.
5. The Contractor must ensure a sufficient number of employees for the smooth and sound implementation of the service.
6. It is recommended that the Contractor make an on-premises vision of the Contracting Authority's facilities in order to estimate on his own responsibility, cost and risk any data that may be necessary to prepare the offer and sign the contract. The deadline for the local vision is set at 16 June 2021:10 a.m.
7. The Contracting Authority shall not allow the submission of partial tenders.
8. The Contracting Authority shall not reserve the obligation of the Contractor to perform the key tasks relating to the contract.
9. The Contracting Authority pursuant to Article 95 (1) of the Act requires that the subject of the contract in respect of maintenance of cleanliness, disinfection and transport operations and other services resulting from the description of the object of the contract and the provisions of the contract be exercised by persons employed by the Contractor or subcontractor on the basis of an employment contract within the meaning of the provisions of the Act of 26 June 1974 — as defined in Article 22 (1) of the Labor Code (Journal of Laws of 2019, item 1040, 1043 and 1495).
1) It is not permissible to replace the employment contract with a civil law contract while maintaining the conditions of performance of the work referred to in Article 22 (1) Kp.
2) If a person terminates an employment relationship before the end of the term of the contract, another person should be hired immediately.
3) In order to document the fulfilment of the abovementioned obligation, the Contractor shall submit to the Contracting Authority, at his request, statements of employment of persons named by name and specifying the scope of their activities.
4) The Contracting Authority shall, in the course of the execution of the contract, have the right to verify the reliability of the statement made by the Contractor, in particular by requesting the submission of a copy certified for compliance with the original of the contracts of employment. If these requirements are not met, the Contracting Authority shall provide for the sanctions listed in the template of the contract.
10. The Contracting Authority does not provide for the employment requirements of persons referred to in Article 96 (2) (2) of the Pzp.
