1. The subject of the contract is the delivery and assembly of equipment for the needs of the Rehabilitation Center in the Kociewie Health Center in Starogard Gdański as part of the project "Extension, reconstruction and retrofitting of Kociewie Health Center Sp. z o.o. in Starogard Gdański and improvement of the qualifications of medical staff in order to increase the availability of specialized health services in the field of civilization diseases" implemented under the Regional Operational Program of the Pomeranian Voivodeship 2014-2020, Priority Axis 7 Health, Sub-measure 7.1.2 Health protection resources.

2. Delivery of equipment to the place indicated by the Ordering Party shall take place by an appropriate means of transport, at the expense and risk of the Contractor.

3. The contractor shall be responsible for the proper preparation of the equipment for operation, commissioning and training of personnel.

4. The description of the subject of the contract (OPZ) constitutes Appendix No. 1 to the SWZ.

5. The Contractor shall grant at least 24 months of warranty and guarantee.

6. The Contractor is obliged to perform the contract on the terms and conditions described in the OPZ constituting Annex No. 1 to the SWZ and in the draft provisions of the contract constituting Annex 9 to the SWZ.
