Subject of the order: “Purchase of a digital telecomando X-ray apparatus with adjustment of the room of the RTG Studio in the Self-Public Health Care Department — Dr. J. Dietl Hospital in Krynica-Zdrój”, which is described in detail in the attached description of the object of the order annex 1 to this Specification of the Terms of the Order.

For a detailed description of the item of the order, see Annex 1 to the SWZ. The Contractors shall, together with the offer, submit the means of evidence in question. To confirm compliance with the criteria set out in the description of the criteria for evaluation of tenders set out in point 21 SWZ, the Ordering Party requires the submission of 1 Folders (original catalogue cards of the manufacturer or authorized distributor in Polish or English) regarding the description of the object of the order — confirming that the products offered comply with the parameters required by the Contracting Authority. Documents described in accordance with the submitted tender — item No, respectively, of Annex 1 to which it relates — In the absence of certain parameters on the catalogue card, the attachment of the equipment maintenance manual or the declaration of the manufacturer or the authorized distributor to the offer of the equipment maintenance manual or the declaration of the manufacturer or the authorized distributor of parameters required. Where an authorised distributor declaration is attached, the appropriate authorisation to represent the manufacturer shall be submitted. On confirmation that the delivery offered satisfies the requirements and characteristics specified by the Contracting Authority, the Contracting Authority shall require the submission of the means of evidence in question:

(a) folders (original catalogue cards of the manufacturer or authorized distributor in Polish or English) regarding the description of the object of the order — confirming that the products offered meet the parameters required by the Ordering Party. Documents described in accordance with the submitted tender — Item No 1 respectively in Annex 1 to which it relates — In the absence of certain parameters on the catalogue card, it is permitted to attach an equipment maintenance manual or a declaration by the manufacturer or authorized distributor to meet required. Where an authorised distributor declaration is attached, the appropriate authorisation to represent the manufacturer should be submitted;

(b) a declaration of conformity with current European Community standards or a notifying body certificate and a CE marking or equivalent in accordance with Annex No 1;

(c) a document proving by the manufacturer to have the authority of the designated service to repair, assemble and servicing the equipment offered in accordance with Annex No 1;

(d) any documents referred to in Annex No 1 (if applicable).
