1. The subject of the order is "Delivery of equipment in accordance with contract no. 1/21/16/2025/305/249 for the implementation of the long-term program titled National Oncology Strategy, regarding the task titled: "Purchase of equipment for pathological diagnostics" in 2025 for SP ZOZ CSK UM in Łódź”
1.1. A detailed description and scope of the subject of the order is specified in Appendix no. 2 to the Terms of Reference (ToR).
1.2. The Employer has divided the order into individual parts and allows the submission of partial offers - Packages: No. 1 – No. 5.
The offered subject of the order must: meet the requirements set by the Employer in the Terms of Reference, in particular the conditions specified in the description of the subject of the order contained in Appendix no. 2 of the ToR. Failure to meet even one of the boundary conditions specified in the aforementioned Appendices will result in the rejection of the offer.

1.4. The implementation of the task under CONTRACT no. 1/21/16/2025/305/249 for the implementation of the long-term program titled National Oncology Strategy, regarding the task titled: "Purchase of equipment for pathological diagnostics" in 2025.
2. Whenever the documentation indicates a brand or origin of the product or device, it should be assumed that for each name there are the words “or equivalent”, meaning that the embedded materials, devices, etc. will have (will be characterized by) all parameters not worse than those described in this documentation, for the given item. 3. Any use in the documentation of definitions and trademarks is only exemplary and serves to define the class and quality of materials. If in the description of the subject of the order there are references to trademarks, patents, or origin, source, or a particular process that characterizes the products or services provided by a specific Contractor, if it could lead to the privilege or elimination of certain Contractors or products, unless justified by the specificity of the subject of the order and the Employer cannot describe the subject of the order using sufficiently precise definitions, and such reference is accompanied by the words “or equivalent”. The Contractor may offer equivalent items. The Employer requires that the materials used, if they are different, have quality and technical parameters not worse than those specified in the subject of the order. The burden of demonstrating the equivalency of the offered item lies with the Contractor.
4. The Employer commits to executing the object of the contract for a minimum of 100% of the gross value of the contract.
5. The deadline for the execution of the contract: within 14 days from the date of signing the contract.
