Description:
Delivery, installation, and commissioning of a 1.5 Tesla magnetic resonance imaging machine and X-ray apparatus with room adaptation for A. Falkiewicz Specialist Hospital in Wrocław, including training and relocation of existing equipment.

1. The subject of the contract is the delivery of a 1.5 Tesla magnetic resonance imaging machine and X-ray apparatus along with room adaptation.
2. The subject of this order is the delivery, installation, commissioning, training, and handover for use of the 1.5T Magnetic Resonance Imaging Machine and X-ray apparatus along with room adaptation based on project documentation for the needs of the A. Falkiewicz Specialist Hospital in Wrocław and the relocation of a densitometer from another hospital building.
3. The scope of the order includes:
1) Execution of construction works according to the developed and approved by the Contracting Authority project documentation and granting a minimum 60-month warranty under the terms specified in § 5 of the draft contract provisions, which constitute no. 7 to SWZ (according to the Contractor's declaration made in the offer);
2) Delivery, installation, and commissioning of the medical devices mentioned in Annex 3 to SWZ;
a) delivery, assembly/installation (at a location indicated by the Contracting Authority, ensuring conflict-free operation and ergonomic use), commissioning, conducting functionality tests, and handing over the equipment that constitutes the subject of the order for operation;
b) conducting acceptance tests in accordance with regulations, enabling the obtaining of permission to operate and use X-ray apparatuses in the medical radiological laboratory;
c) conducting training on the operation and basic maintenance of the delivered equipment for the Contracting Authority's personnel, within a timeline established with the Contracting Authority (in accordance with OPZ);
d) granting a minimum 24-month warranty under the terms specified in § 5 of the draft contract provisions, which constitute no. 7 to SWZ (according to the Contractor's declaration made in the offer).
3) The service of relocating the equipment owned by the Contracting Authority: disassembly, securing all components of the apparatus during the relocation, relocation between facilities using Contractor's resources (human and technical resources, equipment necessary to perform the service will be provided by the Contractor), installation at the destination, calibration and commissioning, as well as conducting a technical inspection.
4. A detailed description of the subject of the order is included in the documentation attached to SWZ:
1) Description of the subject of the order (OPZ) in terms of supply and services – Annex 3 to SWZ,
2) Construction project – Annex 8 to SWZ,
3) Detailed design in the sectors covered by the order – Annex 9,
4) Technical specification for the execution and acceptance of construction works in the sectors covered by the order – Annex 10,
5) Bill of quantities of works covered by the order – Annex 11.
6) Draft contract provisions constituting - Annex 7 to SWZ.
