Description:
The contract involves integrating and expanding the provider's IT systems, including database management, virtualization, server equipment delivery, software licenses, electronic signatures, and hardware replacement, divided into four distinct parts.

The subject of the contract is the integration and expansion of the provider's IT systems - database, virtualization of the environment, delivery of server equipment and software licenses, as well as implementation, integration and expansion of the provider's IT systems - electronic signatures, integration and expansion of the provider's IT systems - hardware base - replacement, divided into 4 parts. The proceedings are conducted in accordance with the Act of September 11, 2019, PZP, (Journal of Laws of 2024, item 1320) – hereinafter referred to as PZP, in the mode of an unrestricted tender with an estimated value exceeding EU thresholds (Article 132 PZP) based on Article 138(4) of the PZP act. The contracting authority informs that the contract is co-financed from the funds of the Lubusz Voivodeship Budget and from European Union funds under the National Reconstruction and Resilience Plan, Component D "Efficiency, accessibility, and quality of the healthcare system", Investment D1.1.2 "Accelerating the processes of digital transformation of healthcare through further development of digital services in healthcare" - project titled "Improving the accessibility and development of digital services in healthcare at the Voivodeship Specialist Hospital in Biała Podlaska", application for funding: KPOD.07.03-IP.10-0106/25. The contracting authority anticipates the cancellation of the proceedings in whole or in part in the event of circumstances mentioned in Article 257(1) of the Public Procurement Law. The subject of the contract is defined in the Common Procurement Vocabulary (CPV) codes: 30200000-1 – Computer devices; 48610000-7 – Database systems; 72263000-6 – Software implementation services. The offer consists of: 1) a completed bid form, in accordance with Appendix 1 to the SWZ; 2) a completed assortment-price form, in accordance with Appendix 2 to the SWZ; 3) a filled table of minimum required technical parameters, in accordance with Appendices 4.1 - 4.4 to the SWZ. The offered equipment must be brand new and produced no earlier than in 2025. The offered subject of the contract must be mass-produced. The contracting authority does not allow offering modified items tailored to the needs of the contract. The contractor will provide a minimum 36-month warranty period for the offered equipment. Contractors may apply for the award of the contract jointly. In such a case: 1) in relation to the participation conditions indicated in §1 point 4), contractors applying for the contract jointly may rely on the capabilities of those contractors who will perform the services for which these capabilities are required. 2) in the case mentioned in point 1) contractors applying for the contract jointly must attach to the offer a statement indicating which services will be performed by individual contractors, following the template in Appendix 7 to the SWZ; 3) the contractors are obliged to appoint a representative to represent them in the procurement proceedings or to represent them in the proceedings and to conclude the contract for public procurement. The power of attorney should be submitted along with the offer. The contracting authority – based on Article 139 PZP – informs that it will first examine and assess the offers in this public procurement procedure, and then qualify the contractor whose offer was rated the highest, regarding the absence of exclusion grounds and meeting the conditions for participation in the proceedings. SUBJECT MATTER OF EVIDENCE 1. Refers to Appendix 4.2 to the SWZ Item 5, Item 20, Item 38: The contracting authority requires the contractor to submit with the offer the result of the SPEC CPU2017 Floating Point test, result SPECrate2017_fp_base for the offered processor obtained from the processor performance table published at http://spec.org in the form of a screenshot or printout of the page with a visible date corresponding to the date of submission of the offer. 2. Refers to Appendix 4.4 to the SWZ: Item 5: The contracting authority requires the contractor to submit with the offer the result of the Passmark test for the offered processor, obtained from the processor performance table published at http://www.cpubenchmark.net/ in the form of a screenshot or printout of the page with a visible date corresponding to the period: from the date of publication of the announcement of the order dated 30.09.2025 to the date of submission of the offer; Refers to Appendix 4.4 Item 12, Item 29, Item 39: The contracting authority requires the contractor to submit with the offer the result of the Passmark test for the offered processor, obtained from the processor performance table published at http://www.cpubenchmark.net/ in the form of a screenshot or printout of the page with a visible date corresponding to the period: from the date of publication of the announcement of the order dated 30.09.2025 to the date of submission of the offer; 3. Refers to Appendix 4.4 item Item 1: The contracting authority requires the contractor to submit at least one of the following certificates confirming that the offered subject of the contract meets the requirements in terms of ergonomics, energy efficiency, and environmental protection: - TCO Certificate, or - EPEAT Certificate, or another equivalent environmental certificate confirming compliance with the criteria for ergonomics, energy efficiency, and minimizing negative impact on the environment. To confirm possession of the required certificate, the following must be attached: - in the case of the TCO certificate – a printout from the website http://tcocertified.com/product-finder, indicating that the offered model has a valid TCO certificate, - in the case of the EPEAT certificate – the certificate or a printout of the entry regarding the offered model in the online catalog http://www.epeat.net, - in the case of an equivalent certificate – documents confirming that the certificate covers at least equivalent environmental criteria. Submission of the above certificate aims to confirm compliance of the offered subject of the contract with the DNSH principle (do no significant harm - "do not cause significant harm" to the environment), also applied to the REPowerEU chapters, in accordance with Article 5(2) of the Act.
