The subject of the contract is a service consisting in carrying out educational and cultural activities as well as training and workshops supporting the introduction of new social functions, i.e. the Local Activity Center for young people and protected housing for people leaving foster care and people with intellectual disabilities in a newly renovated tenement house at 115 Piotrkowska Street in Łódź. The order consists of task 1 and task 2 containing 5 subtasks, for a total of 6 tasks. CONTRACT FOR SOCIAL AND OTHER SPECIFIC SERVICES when awarding which the provisions of the Act of 11 September 2019 - Public Procurement Law (i.e. Journal of Laws of 2022, item 1710, as amended), hereinafter referred to as the PPL Act, applicable to classic contracts, with a value equal to or exceeding the EU thresholds - if the value of the contract expressed in PLN is equal to or exceeds the equivalent of EUR 750,000, Taking into account. changes resulting from Section IV, Chapter 4 of the PPL.

40.1 The Ordering Party does not provide: to conclude a framework agreement; the submission of variants and partial tenders; settlement in foreign currencies; an electronic auction; reimbursement of the costs of participation in the proceedings; the award of contracts pursuant to Article 214(1)(7) of the PPL Act; the right of option under Article 441 of the PPL Act; the requirement or possibility to submit tenders in the form of electronic catalogues or to attach electronic catalogues to the tender; The Contractor shall carry out a local inspection or check by the Contractor the documents necessary for the performance of the contract available on the spot at the Ordering Party.
40.2 Scope and conditions of amendments to the concluded contract - issues relating to the contract are regulated in the model contract - Annex 7 to SWZ.
40.3 The Contracting Authority did not conduct preliminary market consultations before initiating the procedure.
The term of the contract – 7 months from the date of conclusion of the contract.
8.3 Information for Contractors jointly applying for a contract award (e.g. consortium, civil partnerships)
8.3.1 The Contracting Authority in relation to Contractors jointly applying for the award of a contract in relation to the condition regarding technical or professional ability allows the contractors to meet the condition jointly.
8.3.2 Compliance with the conditions for participation in the procedure by Contractors jointly applying for the award of the contract will be verified by the Ordering Party in accordance with Article 117(3) of the PPL Act.
8.3.3 Contractors jointly applying for the award of a contract, together with the offer, submit a statement from which it follows which scope of the subject of the contract will be performed by individual Contractors. This statement is annexed to The Annex. 9 to SWZ.
8.3.4 Contractors who jointly apply for the award of a contract:
8.3.4.1 are obliged to appoint a Representative to represent them in the proceedings or to represent them in the proceedings and to conclude a contract. All correspondence will be addressed exclusively to the entity acting as a proxy.
8.3.4.2 are obliged to submit in the offer a Power of Attorney appointing a Proxy referred to in point 8.3.4.1 of the SWZ. The power of attorney should contain the authority to represent in the proceedings or to represent in the proceedings and conclude the contract. The power of attorney should be provided by the Contractor in electronic form. If the Contractor does not have a power of attorney in electronic form (i.e. with a qualified signature of a person authorized to grant it), and has a power of attorney for a given person only in writing (i.e. with a handwritten signature of a person authorized to grant it), he may submit a digital representation of this document with a qualified electronic signature certifying the compliance of the digital reproduction with the paper document.
The method of certifying the compliance of the digital reproduction with the paper document referred to in the preceding sentence is regulated by the Regulation of the Prime Minister of 30.12.2020 on the method of preparing and transmitting information and technical requirements for electronic documents and electronic means of communication in a public procurement procedure or a competition.
