In the case of an offer by Contractors jointly applying for a contract award (consortium):

1) in the Offer Form, the companies (names) of all Contractors jointly applying for the award of the contract should be indicated;

These declarations confirm the absence of grounds for exclusion and the fulfilment of the conditions for participation in the procedure to the extent that each of the economic operators demonstrates compliance with the conditions for participation in the procedure. The statement of the Contractors jointly applying for the award of the contract submitted on the JEDZ form should be submitted together with the offer under pain of nullity, in electronic form (i.e. in electronic form with a qualified electronic signature).

4) the documents referred to in point 9.3 of the SWZ will be submitted by each of the contractors jointly applying for the award of the contract.

The material scope of the subject of the contract includes the total quantities of works in the field of nursery and seed production as well as forest breeding and protection, fire protection, harvesting and harvesting of wood resulting from Zał. No. 2 to the SWZ, which consists of:

Annex No. 2.1. – the planned size of the works;

Annex No. 2.2. - detailed size of works by groups of activities, activities and locations;

Annex No. 2.3.1. – characteristics of forestry in the field of timber harvesting;

Annex No. 2.3.2. – assortment system of timber harvesting in forestry;

Annex No. 2.3.3. – a list of distances and conditions of wood harvesting;

Annex No. 2.3.4. – list of items with additional difficulties in the field of acquisition and skidding;

Annex No. 2.3.5. – list of items not made available for machine acquisition;

Annex No. 2.3.6. – information on the optimal technology of obtaining wood (potential);

Annex No. 2.4.1. - information on the nature of forest care (zones for mowing weeds);

Annex No. 2.4.2. - a list of items not available for renewal with the help of a planter;

Annex No. 2.4.3. - compilation of positions requiring the bottom of the supplied seedlings

Quality standards relating to all essential features of the subject of the contract are specified in Annex 3 to the SWZ, which consists of:

Incl. No 3.1.' Description of the standard of forestry work technology" adopted by Decision No. 34 of the Director General of the State Forests of 12 May 2022 on the introduction of the "Description of the forest work technology standard" in organizational units of the State Forests together with the changes introduced by amending decisions;

Incl. No. 3.2.Description of the standard of forestry work technology in the Regional Directorate of State Forests in Białystok adopted by Decision No. 11/2022 of the Regional Director of State Forests of 29 June 2022 on the introduction of the Regional Description of the ForestRy Technology Standard.

Specified Annex No. 3.1. to the SWZ and in Annex No. 3.2. to the SWZ, a detailed description of the standard of technology for performing individual forest works (activities) is the basis for determining the quality requirements relating to the elements constituting the subject of the contract (Annex No. 2 to the SWZ). If the activity described in the above-mentioned annexes is not provided for in the description of the subject of the contract (Annex No. 2 to SWZ), then in this respect the provisions of Annex No. 3.1. to SWZ and in Annex No. 3.2 to SWZ do not apply.

LIST OF STATEMENTS AND MEASURES OF EVIDENCE IN ORDER TO DEMONSTRATE THE ABSENCE OF GROUNDS FOR EXCLUSION FROM THE PROCEEDINGS AND FULFILMENT OF THE CONDITIONS FOR PARTICIPATION IN THE PROCEEDINGS

In order to confirm the fulfilment of the conditions for participation in the proceedings referred to in point 7.1. SWZ, the Contracting Authority shall, before selecting the most favourable tender, acting on the basis of Article 126 (1) of the PZP, call upon the Contractor, whose offer has been highest assessed, to submit within the time limit not less than 10 days, current at the date of submission of the following measures of evidence:

(b) evidence specifying whether the Contractor indicated in the list of services to confirm the fulfilment of the condition for participation in the procedure for technical or professional capability in the field of experience has been performed or is duly performed,

The evidence referred to above is references or other documents drawn up by the entity to which the services were performed and in the case of recurring or continuous benefits, and if the Contractor for reasons independent of him is unable to obtain these documents — a declaration by the Contractor; in the case of recurring or continuous benefits, references or other documents proving their proper performance should be issued within the last 3 months (back from the date on which the deadline for submission of tenders expires).
