1. The subject of the contract is the preparation of complete and comprehensive design documentation along with the submission of an effective notification of works not requiring a building permit / obtaining a valid (final) building permit / obtaining a final (final) decision for the implementation of a road investment (ZRID) for the task: "Construction of roads with the necessary infrastructure in the area between Miodowa Street and Pod Lipami Street west of Kasztanowa Street in Piekary Śląskie" - Scope 1 - Municipality and Scope 2 - MPWiK, including the preparation of two complete and comprehensive design documentation - 2 separate scopes:
1) Construction of roads with accompanying infrastructure, construction of a rainwater sewage system with reachers, construction of street lighting and construction of a technological channel - Scope 1 - Municipality,
2) Construction of a sanitary sewage system with connections and construction of a water supply network with connections - Scope 2 - MPWiK.
1. The subject of the contract is the preparation of complete and comprehensive design documentation along with the submission of an effective notification of works that do not require a building permit / obtaining a valid (final) building permit / obtaining a legally binding (final) decision for the implementation of a road investment (ZRID) for the task: "Construction of roads with the necessary infrastructure in the area between Miodowa Street and Pod Lipami Street west of Kasztanowa Street in Piekary Śląskie" - Scope 1 - "MUNICIPALITY" and Scope 2 - "MPWIK", including the preparation of complete and comprehensive design documentation with a division into 2 separate scopes: Scope 1 - Construction of roads with accompanying infrastructure, construction of a rainwater drainage system with reaches, construction of street lighting and construction of a technological channel - Scope "Municipality",
Scope 2 - Construction of a sanitary sewage system with connections and construction of a water supply network with connections - Scope "MPWiK".
Separate contracts will be concluded for both scopes by separate contracting authorities. For Scope 1, the agreement will be concluded and financed by the Municipality of Piekary Śląskie, for Scope 2 the agreement will be concluded and financed by MPWiK w Piekary Śląskie Sp. z o.o.

The offer and attachments to the offer should consist of the following files:
1) completed offer form (Annex 1 to the specification), i.e. offer,
2) the statement referred to in Article 125(1) of the Public Procurement Law - ESPD (according to Appendix 2 to the specification),
3) declaration on not being subject to exclusion pursuant to Article 5k of Regulation 833/2014, as amended by Council Regulation (EU) 2022/576 of 8 April 2022 amending Regulation (EU) No 833/2014 concerning restrictive measures in connection with Russia's actions destabilising the situation in Ukraine (Annex 3 to the specification),
4) a copy or information from the National Court Register, Central Registration and Information on Economic Activity or other relevant register confirming that the person acting on behalf of the contractor is authorized to represent him – if the offer is submitted by a person conducting business activity or an entity entered in the above-mentioned registers. The Contractor is not obliged to submit a document if the contracting authority can obtain it using free and publicly available databases, provided that the Contractor provides data enabling access to these documents,
and if applicable
5) power of attorney to act on behalf of the contractor, if a person other than the person authorized to represent the contractor has been indicated in the document confirming the authorization to represent the contractor,
6) in the case of reliance on the resources of the entity providing resources – the obligation of the entity providing resources referred to in Article 118(3) of the PPL,
7)in the case of reliance on the resources of the entity providing the resources – the statement referred to in Article 125(1) of the Public Procurement Law (PPL) of the entity providing the resources,
8) in the case of submission of a bid by contractors jointly applying for the award of a contract – a document confirming the appointment of a representative in accordance with Article 58(2) of the PPL,
9) in the case of submission of a bid by contractors jointly applying for the award of a contract – a statement referred to in Article 117(4) of the PPL.
Along with the offer, the contractor is obliged to submit a statement of non-exclusion and fulfilment of the conditions for participation in the procedure. The declaration must be made on the Single European Procurement Document (ESPD) form, drawn up in accordance with the model standard form set out in Commission Implementing Regulation (EU) 2016/7 of 5 January 2016 establishing a standard form for the Single European Procurement Document.
