1. The subject of the contract is the provision of mobile telephony services with the supply of mobile phones, modems, accessories with active and configured SIM cards for the Marshal's Office of the Małopolska Voivodship as well as voivodship self-government organizational units and voivodship legal persons, i.e.
a) the Regional Center for Social Policy in Krakow,
b) Krakow Center for Rehabilitation and Orthopedics in Krakow,
c) Małopolska Road Traffic Center in Tarnów,
d) Krakow Office of Geodesy and Agricultural Areas in Krakow,
e) Provincial Labor Office in Krakow,
f) Lesser Poland Entrepreneurship Center in Krakow,
g) Provincial Road Administration in Krakow,
h) the Marshal's Office of the Lesser Poland Voivodship,
2. The services will be provided through the use of active SIM cards provided by the Contractor.

2. The services will be provided through the use of active SIM cards provided by the Contractor (maintaining the existing voice numbering of the Employer)
In voice tariffs, date and web browser. The service is to provide, among others: voice (outgoing and incoming), text (SMS), multimedia (MMS) and access to Internet resources.
3. The Employer provides for the transfer of individual numbers to the new Contractor after the end of existing contracts and the expiry of the obligations arising therefrom.
4. The terms of the contract are contained in the contract template constituting Annex 3 to the ToR.
5. A detailed description of the subject of the contract is Annex 4 to the ToR (which is also Annex 1 to the model contract).
6. The contract is co-financed by the European Union from the European Social Fund under the Regional Operational Program of the Malopolska Voivodship 2014-2020.
7. Designation according to CPV: 64212000-5 - Mobile telephony services, 72400000-4 - Internet services, 32250000-0 - Mobile telephones, 32552410-4 - Modems.
8. If the descriptions of the subject of the order include: names of trademarks, patents or origin, this should be treated only as an aid in the description of the subject of the order. In any case, equivalent products in terms of construction, parameters and purpose are allowed.
9. The Purchaser in accordance with the instructions of Art. 29 paragraph 3a of the Act does not require that the person / persons participating in the performance of the contract, be / were employed by the contractor or subcontractor under an employment contract within the meaning of the provisions of the Act of 26 June 1974 - Labor Code.
