1. The subject of the contract is the provision of a service of successive collection and management of waste with the code 19 12 12 – other waste (including mixed substances and objects) from mechanical treatment of waste other than those listed in 19 12 11 – ground bulky waste, generated in the Municipal Waste Disposal Plant in Długoszyn near Sulęcin. 2. The estimated amount of waste to be transported is 1,600 Mg (for a period of 6 months from the date of signing the contract). 3. The Ordering Party reserves that the specified quantity of the subject of the contract may be reduced. The change will oscillate within the limits of - 20 percent of the basic order volume. The above does not result in withdrawal from the contract or change of its terms. The actual amount of waste will result from the current needs of the Ordering Party.

Waste transport: a) Waste collection will take place from the area of the Municipal Installation run by the Special Purpose Association of Municipalities CZG-12, Długoszyn 80, 69-200 Sulęcin, Lubuskie Province, Sulęcin County, Sulęcin Commune. b) Transport of waste will be carried out at 1-2 trips a day, from Monday to Friday, excluding public holidays, from 7:00 to 16:00. The Ordering Party allows the possibility of transporting waste also on Saturdays after prior agreement between the parties. The estimated monthly amount of waste to be collected is approx. 300 Mg, while the estimated daily amount of waste to be collected is approx. 20-40 Mg. c) Waste collection will take place at the request of the Ordering Party in accordance with the collection schedule agreed with the Contractor, specifying the day of collection and the number of trips. (d) Transport of waste shall be carried out in such a way as to prevent the spread of waste outside the means of transport, in particular its discharge, dusting and leakage, and to minimise odour nuisance. e) Waste will be transferred in loose form. f) Transport of waste will take place in a manner that minimizes the impact of atmospheric factors on waste, if they may cause a negative impact of transported waste on the environment or human life and health. g) Loading waste onto vehicles substituted by the Contractor is the responsibility of the Ordering Party. h) Loading of the semi-trailer will be carried out with a loader from above. 5. Requirements for means of waste transport: a) truck - tractor unit, b) semi-trailer with a movable floor opening at the rear, c) semi-trailer capacity - min. 92 m 3, d) semi-trailer height – max. 4 m, e) unloading time of a full semi-trailer – max. 30 minutes, f) load capacity for waste 19 12 12 – approx. 17 Mg, g) the permissible total weight of the combination (tractor unit + semi-trailer with goods) must not exceed 40 Mg. 6. The collected waste with the code 19 12 12 will be managed on the Contractor's installations with a legally valid decision for the processing of the above-mentioned waste in the amount consistent with the subject of the contract. The installation shall present to the Contracting Authority a legally valid decision for waste processing with the code 19 12 12. 7. The Contractor shall be solely liable for any damage, as well as for loss, loss and damage to waste caused during the transport of waste from the moment of delivery to ZUOK in Długoszyn, during transport, to unloading waste. 8. The Contractor shall be liable, in particular, for the acts and omissions of its staff and entities used to perform the subject of the contract. 9. Upon release of waste to the Contractor (to the Carrier indicated by him), all benefits and burdens associated with them and the risk of their accidental loss or damage shall pass to the Contractor, in particular he shall take responsibility for the waste dispensed, for its proper handling and for the consequences resulting therefrom.
