County Durham and Darlington NHS Foundation Trust are looking for a Pain Management Service.
The Pain Management Service should provide a co-ordinated interdisciplinary service that is customised to meet the complexity of needs of people with chronic pain problems in order to restore their functional ability and enable them to live as independently as possible. The service is aimed to allow self-management of pain by giving the patients the necessary support and skills to allow them to manage their own condition away from the need of on-going support.
The service must include an element of patient education in support of self-management. The education should allow the patient and suitable stakeholders the knowledge to understand their condition and allow a more successful self-management.
The Pain Management Service will be led by a clinician with particular skills and experience in the management of chronic pain.
The co-ordinated approach offered by a Pain Management Service interdisciplinary team will use a number of different modalities from a biomedical, psychosocial and functional viewpoints, the service will include as a minimum:
– acupuncture,
– bowen therapy,
– counselling,
– exercise coaching,
– Pain Management plan groups,
– physiotherapy,
– clinician led Pain Management Assessment.
The service must have suitable premises within the area of County Durham which allows suitable access for all patients. The patients will be referred by County Durham and Darlington Foundation Trust.
The service will initially be for a period of 12 months +1 year +1 year extension.
