The subject of the contract is the provision of services on the route Chobot - Nowa Wieś - Niepołomice - Wieliczka - Krakow in the field of public transport of public utility on the following route:
1) Chobot Leśniczówka - Wola Zabierzowska- Nowa Wieś- Zabierzów Bocheński- Wola Batorska- Niepołomice Dworzec- Niepołomice Rynek- Niepołomice ul. T. Kościuszki- Podłęże- Wieliczka- Kraków ul.
The subject of the contract is the provision of services on the route Chobot - Nowa Wieś - Niepołomice - Wieliczka - Krakow in the field of public transport of public utility on the following route:
1) Chobot Leśniczówka - Wola Zabierzowska- Nowa Wieś- Zabierzów Bocheński- Wola Batorska- Niepołomice Dworzec- Niepołomice Rynek- Niepołomice ul. T. Kościuszki- Podłęże- Wieliczka- Kraków ul. Starowiślna- with the final stop of Kamienna 8 Street Departure from the bus terminus in Chobot from Monday to Friday at 05:05, last departure at 18:00 Departure from Krakow from 06:30, last departure at 19:40 from Monday to Friday.
1. The Contractor shall complete all formalities related to the commissioning and operation of the line on the Chobot-Kraków route.
2. The Contractor shall charge for a normal ticket:
1) in the amount of PLN 6.50 (six zlotys 50/100) per passenger for one course on the Chobot-Kraków route,
2) in the amount of PLN 6.00 (six zlotys 00/100) per passenger for one course on the Chobot-Wieliczka route
3) in the amount of PLN 5.50 (five zlotys 50/100) per passenger for one course on the Niepołomice Wieliczka route,
4) in the amount of PLN 5.00 (five zlotys 00/100) per passenger for one course on the route Niepołomice- Podłęże
5) in the amount of PLN 4.00 (four zlotys 00/100) per passenger for one course on the Chobot-Niepołomice route
Eligible for free travel are school youth from primary schools with a valid school ID. Other persons entitled to discounts for travel are included in the regulations that apply in public transport at MPK in Krakow - the list of those entitled to travel is included in the annex.
3. Income from the sale of tickets will constitute additional remuneration of the Contractor.
4. Planned number of trips per day on weekdays - 9
5. Planned number of courses per week to - 45
6. The length of the route Chobot- P&R Podłęże- Chobot - 85,4km
7. For the purpose of calculating the price, the number of trips in the year should be taken - 2.277
8. For the purpose of calculating the price, the number of kilometers should be taken - 194,455.80 km
THE REQUIREMENTS REFERRED TO IN ARTICLE 95 PZP.
Since the performance of this contract must take place by performing activities in the conditions specified in Article 22 § 1 of the Act of 26 June 1974 Labour Code - for the benefit of the contractor or subcontractor, at the place and time indicated by the contractor or subcontractor - the contracting authority, in accordance with Article 95 paragraph 1 of the PPL Act, requires employment by the contractor or subcontractor of persons performing activities included in the so-called direct costs on the basis of the employment relationship. This requirement applies to persons who perform activities directly related to the performance of services covered by the subject of the contract, i.e. the so-called manual workers, in particular:1) drivers,
2. The method of documenting the employment of persons referred to in Article 95 of the PPL Act. The Contractor is obliged to submit to the contracting authority, at its request within the time limit specified in the application, documents confirming the employment of persons performing the activities referred to above, in particular a written statement of the contractor or subcontractor confirming that the contractor's employees or subcontractor are employed under an employment contract within the meaning of the provisions of the Act of 26 June 1974 - Labor Code, taking into account the minimum wage for work determined on the basis of art. 2 par. 3-5 of the Act of 10 October 2002 on the minimum wage for work (Journal of Laws of 2020, item 2207). This statement should contain, in particular: the exact determination of the entity submitting the declaration, the date of submission of the declaration, an indication that the activities listed in point 1 are performed by persons employed under an employment contract together with an indication of the number of these persons, the type of employment contract and the length of employment and the signature of the person authorized to make a statement on behalf of the contractor or subcontractor.
