1. The subject of the contract is the delivery of medical equipment and equipment as part of the implementation of the task: “Purchase of medical equipment and equipment for the operating block of the Multispetialist District Hospital S.A. in Tarnowskie Góry”. The order was divided into 5 parts:

2. The detailed scope of the subject of the contract, the description of the requirements of the Contracting Authority for execution and collection specify:
1) the price form together with the description of the subject of the order in Annex 2 to the SWZ,
2) Designed terms of the contract — Annex 3 to the SWZ.
Part A — anesthesia columns1. The subject of the contract is the delivery of medical equipment and equipment as part of the implementation of the task: “Purchase of medical equipment and equipment for the operating block of the Multispetialist District Hospital S.A. in Tarnowskie Góry”. 2. The detailed scope of the subject of the contract, the description of the requirements of the Contracting Authority for execution and collection specify:
1) the price form together with the description of the subject of the order in Annex 2 to the SWZ,
2) Designed terms of the contract — Annex 3 to the SWZ.
3. The required warranty period and warranty for the executed object of the contract shall be a minimum of 24 months from the date of signing by the Ordering Party, the Investor Supervision Inspector and the Contractor of the fault-free delivery protocol.
4. The contracting authority shall request from the contractor referred to in the description of the subject of the contract annex 2 to the SWZ in question proving that the deliveries offered meet the requirements specified by the Contracting Authority.
5. The Contractor shall submit the means of evidence in question together with the offer.
6. If the contractor has not submitted the means of evidence in question or the evidence submitted in question are incomplete, the contracting authority shall call for them to be lodged or supplemented within the deadline set.
7. The call procedures referred to in paragraph 6 shall not apply if the measure of evidence in question serves to confirm compliance with the characteristics or criteria set out in the description of the criteria for evaluating tenders, or despite the submission of the measure of evidence in question, the tender shall be rejected or may occur the premises for annulment of the proceedings.
8. The contracting authority may request clarification from the contractors regarding the content of the evidence measures in question.
