Hampshire Partnership and West Hampshire CCG have identified a cohort of People with Learning Disabilities and complex needs (including Forensic history, autism, mental health and with behaviours that challenge) who are currently undergoing rehabilitation care in hospital where there is the opportunity to transfer this care to the community setting. It is anticipated that these people would be eligible to claim Housing Benefit and will require support services (domiciliary care).The list is quite extensive, so for demand management purposes the plan is to first prioritize those people who have had a hospital length of stay equal or greater than 5 years. This need has been the basis for an NHS England capital bid application of a total 250 000 GBP, to identify suitable Registered Providers who could supply new/refurbished premises (minimum of five flats), that could add to the existing supported living accommodation profile for people with Learning Disability across Hampshire.
