Successive deliveries of poultry, red meat, animal fats, red meat sausages and poultry sausages in 2023 to the 41. School Aviation Base in Dęblin - No. 31/23/N

The subject of the contract are successive deliveries of poultry, red meat, animal fats, red meat sausages and poultry sausages in 2023 to the 41st Annual General Meeting of the European Development Fund. School Aviation Base in Dęblin. Deliveries must be made in accordance with the standards and assortment, quantities and requirements set out in Chapter IV of the SWZ and with the quality requirements specified in the "Minimum quality requirements" constituting the "Detailed description of the subject of the contract" for individual tasks - Annexes 2a, 2b, 2c to the SWZ. The terms and conditions for the performance of this contract are also specified in the "draft provisions of the public procurement contract that will be introduced into the public procurement contract" for individual tasks - Annex 5/5a/5b to the SWZ. The contracting authority allows for the possibility of submitting partial bids for 1, 2 or 3 lots of the contract. Delivery of goods - successively - three times a week: Monday, Wednesday, Friday from 8.00 a.m. to 10.00 a.m.

Task No I: Successive delivery of poultry

Task No. I - Successive deliveries of poultry in 2023 - Quantities constituting the basic order (guaranteed to the Contractor):

The offer must be secured by a deposit. The tendering security must be lodged before the closing date for the submission of tenders. The Contractor is obliged to pay a deposit in the amount of: Task No. I – PLN 1000.00. If the contractor does not submit the evidence in question together with the bid or the evidence submitted is incomplete, the contracting authority shall call for their submission or supplementation within the prescribed period. The contracting authority requires the contractor whose tender has been selected as the most advantageous in a given task (part) of the contract to lodge a performance bond in the amount constituting 3% of the contractor's gross remuneration covering deliveries carried out under the basic contract

The selected Contractor, with whom the Ordering Party signs a public procurement contract for a given task, must prove to be in possession of civil liability insurance in the entire period of performance of the contract in the scope of business activity related to the subject of the contract for a guarantee amount not less than: for TASK NO. I – PLN 20,000.00, together with proof of payment of premiums.

For each task: Information on the evidence in question:

1. The Contracting Authority, in connection with the description of the subject of the contract by means of standards – allows solutions equivalent to those described in Annex 2a to the SWZ The Contractor, who refers to solutions equivalent to those described by the contracting authority, is obliged to demonstrate that the deliveries of the subject of the contract offered by him meet the requirements specified by the contracting authority in Annex 2a to the SWZ ("Minimum quality requirements" containing detailed descriptions of the subject of the contract for task No. 1), submitting together with the offer: - a description of each equivalent product, e.g. a detailed product card, a declaration of conformity, a protocol of laboratory tests, confirming the compliance of the physico-chemical, quality properties of a given product in relation to those described by the ordering party. The description of each equivalent product shall be attached with the tender only by the Contractor who offers equivalent products.

2. The offered deliveries meet the requirements specified by the Contracting Entity: in particular, the Contractor has implemented and applies the implemented HACCP system. In order to confirm that the offered deliveries meet the requirements and characteristics specified by the Ordering Party, the Contracting Authority requires the submission of the following means of proof together with the tender: The Contractor shall demonstrate that it has a valid document confirming the implementation of the HACCP system: HACCP certificate issued by an independent, accredited certification body or a certificate of the competent authority of the State Sanitary Inspection or the competent authority of the Veterinary Inspection on the exercise of supervision over the implementation of the HACCP system. The certificate submitted on this basis should confirm that the contractor has implemented the principles of the HACCP system or the protocol of sanitary control carried out by the State Sanitary Inspection authorities as an official food control body, which includes a point containing a positive assessment of the development, implementation and maintenance of the HACCP system on the basis of the Act of 25 August 2006 on food and nutrition safety or other documents confirming the use of The contractor of equivalent quality assurance measures and the application of equivalent management measures. The Contractor is obliged to submit the said evidence together with the offer. If the contractor does not submit them together with the bid or the evidence submitted is incomplete, the contracting authority will call for their submission or supplementation within the prescribed period.

The bid must be secured with a bid bond in the amount of: Task No. II – PLN 6100.00 (six thousand one hundred zlotys) The bid bond must be lodged before the deadline for submitting tenders. The contracting authority requires the contractor whose tender has been selected as the most advantageous in a given task (part) of the contract to lodge a performance bond in the amount constituting 3% of the contractor's gross remuneration covering deliveries carried out under the basic contract

The offer must be secured with a bid bond – for Task No. III in the amount of PLN 4500.00 (four thousand five hundred zlotys). The tendering security must be lodged before the closing date for the submission of tenders. The contracting authority requires the contractor whose bid has been selected as the most advantageous in a given task (part) of the contract to lodge a performance bond in the amount constituting 3% of the contractor's gross remuneration covering supplies carried out under the basic contract. The Contractor selected for a given task must prove to have throughout the entire period of the contract civil liability insurance in the scope of business activity related to the subject of the contract for a guarantee sum not less than: for TASK NO. III – PLN 100,000.00, together with proof confirming the payment of premiums. For each of the tasks: Information on the evidence in question: The contracting authority, in connection with the description of the subject of the contract by means of standards – allows solutions equivalent to those described in Annex 2c to the SWZ The contractor who refers to solutions equivalent to those described by the contracting authority is obliged to demonstrate that the supplies offered by the contracting authority meet the requirements specified by the contracting authority in Annex 2c to the SWZ ("Minimum quality requirements" - detailed descriptions of the subject of the contract for task No. 3), submitting together with the offer: - description of each equivalent product, e.g.: detailed product sheet, declaration of conformity, laboratory test report, confirming the compliance of physico-chemical and qualitative properties of a given product in relation to those described by the Ordering Party. The description of each equivalent product shall be attached with the tender only by the Contractor offering equivalent products. The offered deliveries meet the requirements specified by the Contracting Entity: in particular, the Contractor has implemented and applies the implemented HACCP system. In order to confirm that the offered supplies meet the requirements specified by the Contracting Entity, it is required to submit the following means of proof together with the tender: The contractor shall demonstrate that it has a valid document confirming the implementation of the HACCP system: HACCP certificate issued by an independent, accredited certification body or a certificate of the competent authority of the State Sanitary Inspection or the competent authority of the Veterinary Inspection on the exercise of supervision over implementation of the HACCP system.
