1. The subject of the contract is the supply of milk and dairy products for the 31st Tactical Air Base.

2. A detailed description, applicable documents and quality requirements referred to in Article 246(2) of the PPL are included in Annex 7 and Annex 4 to the SWZ. The scope of the contract is included in Annex 1a to the SWZ - price form.

3. Delivery of goods should be carried out between 0700 and 0900 – 3 times a week

i.e.: Monday, Wednesday, Friday, in accordance with the applicable quality standards and procedures of the HACAP system (quality and transport requirements) and in the packaging indicated by the Ordering Party in Annex 7 to the SWZ and Appendix No. 1 to the contract, under pain of refusal to accept it. If the specified delivery day falls on a non-working day, the delivery must be made on the day preceding the non-working day or on another date agreed in writing between the Contractor and the representative of the Ordering Party.

1. In order to confirm that the offered supplies meet the requirements and characteristics specified by the Ordering Party, the Ordering Party requires the submission of the following means of proof:

Current document confirming the use of the implemented HACCP system:

a) certificate of the HACCP system implemented by companies accredited and authorized by the Polish Centre for Accreditation based in Warsaw,

or

b) Certificate of the competent authority of the State Sanitary Inspection or the competent authority of the Veterinary Inspection on supervising the application of the principles of the implemented HACCP system.

The submitted certificate should confirm that the Contractor has implemented and applies the principles of the HACCP system – Legal basis: Articles 59 and 73 of the Act of 25 August 2006 on food and nutrition safety.

2. The Contractor shall be obliged to submit the said means of proof together with the tender.

3. If the Contractor fails to submit the evidence in question or the evidence submitted is incomplete, the Contracting Authority shall call for their submission or supplementation.
