Description:
Development of comprehensive documentation for the investment task entitled: "Expansion of the provincial road No. 837 on the section from km approx. 6+141 to km approx. 20+300."

The subject of the contract is: Development of comprehensive documentation for the investment task entitled: "Extension of the voivodship road No. 837 on the section from km approx. 6+141 to km approx. 20+300."

1. The basic purpose of the order is:

a) Development of multi-branch comprehensive design documentation at the stage of Construction and Executive Projects together with geodetic and legal documentation, decisions and all arrangements and opinions, for the project entitled "Extension of the voivodship road No. 837 on the section from km approx. 6+141 to km approx. 20+300" enabling submission of an application and obtaining a decision on permission to implement a road investment (ZRID decision)

pursuant to the Act of 10 April 2003 on specific rules for the preparation and implementation of investments in the field of public roads (Journal of Laws of 2022, item 176, as amended) together with obtaining the required opinions on the application and preparing the application together with materials for the application for obtaining this decision.

(b) Development of materials and obtaining the attachments necessary to obtain a decision on the environmental conditions with its obtaining by the Contractor.

c) Development of materials and obtaining attachments necessary to obtain a water permit / water law consent along with obtaining it by the Contractor.

(d) Development of the materials necessary for the announcement of a tender for the performance of the construction works required by the Public Procurement Law and subsequent implementation of the task.

2. The Contractor shall provide the Author's Supervision during works carried out on the basis of design documentation prepared on the basis of this contract and to the extent specified by the Construction Law (Article 20 paragraph 1 point 4 of the Act [1]) and in accordance with the arrangements set out in the Agreement. The Author's Supervision will take place on the basis of the right of option within the meaning of Article 441 paragraph 1 of the Public Procurement Law. The order to perform the Author's Supervision will depend on the needs of the Ordering Party.

The Contractor is obliged to exercise the Author's Supervision during the execution of works covered by the design documentation, the planned date of completion of the investment is 31 December 2027.

3. The detailed scope of the subject of the contract is included in the "Description of the Subject of the Contract" (hereinafter referred to as "OPZ" constituting an attachment to the SWZ.)

The contracting authority in OPZ specified that the subject of the contract, i.e. the project, is to include, m.in. "Removal of architectural barriers within the extended road section to ensure accessibility for all users, in particular for the disabled" (point 2.12 OPZ), and thus as a public facility after the completion of the extension, it will not have accessibility restrictions by all road users, including the disabled, which is required by the provisions of the Regulation of the Minister of Infrastructure on technical and construction regulations regarding public roads (Journal of Laws of 2022, item 1518) and the Regulation of the Minister of Infrastructure on technical conditions to be met by buildings and their location.

At pedestrian crossings and bus stop bays, elements should be used to facilitate perception by blind and visually impaired people (concrete blocks with tabs).

4. In accordance with Article 13(1) and (2) of Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and in

on the free movement of such data and repealing Directive 95/46/EC, The General Data Protection Regulation (OJ L ..., p. EU L 119, 4.5.2016, p. 1), hereinafter referred to as "GDPR", informs that:
