The subject of the contract are services in the field of forest management as defined in Article 6(1)(1) of the Forest Act of 28 September 1991 (consolidated text: Journal of Laws of 2023, item 1356 – "Forest Act") including works in the field of forest breeding and protection, fire protection, timber harvesting and harvesting to be performed in the Bielsk Forest District in Bielsk Podlaski in the years 2024-2025.

Quality standards relating to all essential features of the subject of the contract are specified in Annex 3 to the SWZ "Description of the standard of forestry work technology".
A detailed description of the standard of the technology for the performance of individual forestry works (activities) specified in Appendix No. 3 to the Terms of Reference is the basis for determining the quality requirements relating to the elements constituting the subject of the contract (Appendix No. 2 to the Terms of Reference). If the activity described in the above-mentioned attachment is not provided for in the description of the subject matter of the contract (Appendix No. 2 to the Terms of Reference), then the provisions of Appendix No. 3 to the Terms of Reference do not apply in this respect

LIST OF STATEMENTS AND MEASURES OF EVIDENCE IN ORDER TO DEMONSTRATE THE ABSENCE OF GROUNDS FOR EXCLUSION FROM THE PROCEEDINGS AND FULFILMENT OF THE CONDITIONS FOR PARTICIPATION IN THE PROCEEDINGS
In order to confirm that there are no grounds for exclusion from the procedure referred to in paragraphs 6.1 - 6.4 of the Terms of Use and to confirm the fulfilment of the conditions for participation in the procedure referred to in clause 7.1. The Contractor is obliged to submit the following along with the offer:
(a)a declaration by the Contractor that it is not subject to exclusion and fulfils the conditions for participation in the procedure, submitted on the Form of the European Single Procurement Document ('JAD'), drawn up in accordance with the model standard form set out in Commission Implementing Regulation (EU) 2016/7 of 5 January 2016 establishing a standard form for the European Single Procurement Document (OJ L ..., p. EU L Series 2016 No. 3, p. 16). The JEDZ is evidence confirming the absence of grounds for exclusion and the fulfilment of the conditions for participation in the procedure, as at the date of submission of tenders, temporarily replacing the subjective means of proof required by the Ordering Party.
b) the Contractor's statement on the lack of grounds for exclusion in the scope referred to in Article 5k of Council Regulation (EU) No 833/2014 of 31 July 2014 – (the template of the Contractor's statement regarding the grounds for exclusion from Article 5k of Regulation 833/2014 constitutes Annex 5 to the SWZ)
