The subject of the contract is the purchase and implementation of equipment as part of the project entitled "Implementation of an integrated medical IT system at the Center for Pulmonology and Thoracic Surgery in Bystra in order to increase access to digital public services in the area of e-health in the Silesian region", co-financed from European funds under the Regional Operational Program of the Silesian Voivodeship 2014-2020.
The subject of the contract is the purchase and implementation of equipment as part of the project entitled "Implementation of an integrated medical IT system at the Center for Pulmonology and Thoracic Surgery in Bystra in order to increase access to digital public services in the area of e-health in the Silesian region", co-financed from European funds under the Regional Operational Program of the Silesian Voivodeship 2014-2020.
The subject of this contract includes the supply of the following hardware with software and licensing:
1) Server Type 1 – 2 pcs.
2) Server Type 2 – 2 pcs.
3) Matrix – 1 pc.
4) NAS – 1 pc.
5) Switch Type 1 – 1 pc.
6) Switch Type 2 – 2 pcs.
7) License for virtualization software – 1 set
8) Backup software license – 1 set
9) License for the server operating system – 8 kpl.
The subject of the contract also includes installation and implementation services within the scope of the subject of the contract.
Detailed requirements regarding the subject of the contract - functional and implementation are specified in Annex 4 to SWZ - Detailed Description of the Subject of the Order and in Annex A.01 to A.09 to Annex. No. 4 SOPZ.
Warranty conditions: The ordering party requires a guarantee on the terms set out in the provisions of the SWZ. The Ordering Party requires a guarantee for the subject of the order for a period of 5 years from the date of signing the final acceptance protocol, as well as the removal of all reported defects of the subject of the order, even despite the end of the warranty period, if they were reported before the end of the warranty period. The warranty on the subject of the contract is valid for a period of 60 months from the date of signing the final acceptance protocol.
The contracting authority in this procedure does not provide for the use of the so-called reverse procedure, referred to in Article 139(1) of the PPL Act.
The Ordering Party shall allow equivalent solutions described in detail in the SWZ in Chapter II. All solutions that meet the minimum criteria for individual assortments indicated in the SOPZ will be considered equivalent. The materials and devices described in the order documentation are the standard expected by the Ordering Party. The Ordering Party allows the use of materials and devices – equivalent (in the case of specifying a trademark or manufacturer), but with parameters and quality not inferior to those possessed by the materials and devices proposed by the Ordering Party. In the event that the detailed description of the subject of the contract (Annex No. 4 to SWZ - SOPZ and in the Annex to SOPZ in Annex A.01 to A.09 constituting Annex 4) contain references to standards, technical assessments, technical specifications and technical reference systems referred to in Article 101(1)(2) and (3) of the PPL Act, The Ordering Party shall allow solutions equivalent to those described. In this case, any references indicated to technical assessment standards, technical specifications and technical reference systems shall be read together with the words 'or equivalent'.
Obligation to take into account accessibility requirements for persons with disabilities or design for all participants: As part of the proceedings, websites or applications containing information indicated in Article 8(2)(1) and (2) of the Act of 4 April 2019 on the digital accessibility of websites and mobile applications of public entities (Journal of Laws of 2019, item 848) will not be created or implemented. Therefore, the provisions of this Act do not apply.
