The subject of the contract is the execution of construction works consisting in the renovation of the pavement of selected streets located in the City of Łódź in the following locations:

- street coolers,

- street Deciduous,

- street Tadeusz Boy-Żeleński,

- street Lechicka,

- street limestone,

- street Vilnius.

A detailed description of the subject of the contract is included in the price forms of parts 1-6 (Forms 2a - 2f) - Volume I of the ToR, in the Technical Specifications of Construction Works Performance and Acceptance - Volume II of the ToR and in Volume III of the Terms of Reference - Contract Act Form.

1. The scope of works includes renovation of pavement surfaces and shoulders of ul. Chłodnikowa from No. 13 to No. 39a.

2. A detailed description of the subject of the contract is included in the Price Form (Form 2a) - Volume I of the ToR, in the Technical Specifications of Performance and Acceptance of Construction Works - Volume II of the ToR and in Volume III of the Terms of Reference - Contract act form.

3. Employer, pursuant to art. 29 paragraph 3a of the Act, requires that the performance of the contract consisting in driving or operating road equipment should be performed by at least 3 people employed by the Contractor or subcontractor under an employment contract.

4. Employment requirements by the Contractor or subcontractor on the basis of an employment contract referred to in art. 29 paragraph 3a of the Act, persons performing activities in the field of the contract are specified in item 3.6. Volume I SIWZ and in Volume III SIWZ - Contract act form

The above requirements specify in particular:

a) the method of documenting the employment of persons referred to in art. 29 paragraph 3a of the Act,

b) the Awarding Entity's powers to control compliance of the contractor with the requirements referred to in art. 29 paragraph 3a of the Act and sanctions for failure to meet these requirements,

c) the type of activities necessary to perform the contract to which employment requirements relate, based on an employment contract by the contractor or subcontractor of the person performing the activities during the performance of the contract.

1. The scope of works includes renovation of the surface of ul. Liściesta from ul. Mieczysław Jagoszewski towards ul. Szczecin.

2. A detailed description of the subject of the contract is included in the Price Form (Form 2b) - Volume I of the ToR, in the Technical Specifications of Construction Works Performance and Acceptance - Volume II of the ToR and in Volume III of the Terms of Reference - Contract Act Form.
