Sandwell and West Birmingham Hospitals NHS Trust provides a range of nurse led bed based intermediate care services for the Sandwell and West Birmingham CCG population. The key aim of bed based intermediate care is to provide a multidisciplinary approach to a time limited period of safe, effective assessment and rehabilitative support with the opportunity for active recovery in order to achieve lowest most stable level of on-going need. This will include provision for those where:

— the intention is for them to return to their own home/ usual place of residence;

— Those requiring step down from acute care receive assertive case management to support safe discharges;

— those who require a period of enhanced assessment where it is not clear whether they can remain at or return home and require a complete review of their care needs and care arrangements.

The bed based facilities will provide a genuine alternative to hospital admission for people who can be managed in the community. They operate as a step down facility to facilitate timely hospital discharge and step up facility to prevent unnecessary hospital admission. Both aims require the provision of opportunities for assessment, treatment and or rehabilitation.

We have a requirement for a full range of GP services for a Nurse led, bed based, intermediate care service.

The current services are provided over three sites, all require Medical services, most on a daily basis, to support the provision of safe effective services, we propose to separate out the following into three lots, potential bidders can bid for 1, 2 or all 3 lots:

— Lot 1. Leasowes Intermediate Care Centre — 18 beds general rehabilitation, 2 dedicated beds for home from home end of life care;

— Lot 2. City Hospital site — Sheldon Unit. 2 wards — D43 unit, 27 beds for patients deemed medically fit for discharge, from acute hospital care, to allow for assertive case management and effective discharge planning. D47 20 flexible beds for intermediate care, to promote recovery, both step up from home, and step down from acute care;

— Lot 3. Rowley Regis Hospital — Eliza Tinsley Unit — 24 beds for patients deemed medically fit for discharge, from acute hospital care, to allow for assertive case management and effective discharge planning. 24 beds for intensive intermediate care, to promote recovery, both step up from home, and step down from acute care.
