Description:
The project involves design and construction works for the modernization of building K at St. Wojciech Hospital in Gdańsk, focusing on enhancing cardiology care through the installation of diagnostic equipment including an angiograph and magnetic resonance imaging.

1. The subject of the order includes, among others, the execution of design works and construction-installation works along with the delivery of the angiograph and preparation of premises for the installation of magnetic resonance imaging, involving the adaptation of the premises of building K in the St. Wojciech Hospital in Gdańsk to improve the accessibility and quality of cardiology care. As part of this investment, the reconstruction and modernization of part of the premises and hospital infrastructure located on level 0 of building K and A is planned. The procedure is in the design and build formula. 2. The subject of the order consists of one part. The detailed scope and method of execution of the subject of the order is specified in Annex No. 2 to the SWZ - Description of the Subject of the Order (referred to as OPZ) along with attachments. The conditions for the execution of the order are contained in Annex No. 4 to the SWZ - Draft agreement with attachments. 3. The Contractor will provide a guarantee for the construction and installation works covering the subject of this order for a period of at least 36 months, a maximum of 60 months (from the date of final acceptance of the works). Definitions of acceptance and guarantees are described in Annex No. 4 to the SWZ. The warranty period is one of the criteria for evaluating offers. 4. The Contractor is obliged to conclude, for the duration of the agreement regarding this order, no later than the day preceding the day on which the site will be handed over, an insurance agreement or agreements for all risk and liability related to the execution of the agreement, and to timely pay the due insurance premiums and pay within the deadline specified in the policy, as described in point No. 16 of Annex No. 4 to the SWZ. 5. Delivery of the angiograph:

  • The detailed description regarding the purchase of the angiograph is included in Annex 2 to the Agreement - Detailed specification of medical equipment, which the Contractor completes and attaches to the offer. By filling in the indicated items, the Contractor confirms meeting the requirements set by the Ordering Party,
  • The Contractor will provide a guarantee for the angiograph for a period of 36 months,
  • It is specified that the offered items of the order must be brand new, complete, of high standard both in terms of quality and functionality, as well as free from material, structural, and legal defects,
  • The subject of the order must comply with the Act of April 7, 2022, on medical devices (i.e. Journal of Laws 2022.974 of 2022.05.09).
6. Equivalent solutions: In all provisions of the SWZ and its attachments that indicate trademarks, parameters, sources or a particular process characterizing products supplied by a specific contractor (names of manufacturers or devices), in accordance with Art. 99 sec. 5 of the Pzp and when describing the subject of the order by reference to standards, technical assessments, technical specifications, reference systems, in accordance with Art. 101 sec. 4 of the Pzp Act, the Ordering Party allows for equivalent solutions. 7. The Ordering Party does not reserve the possibility of applying for the order solely by Contractors referred to in Art. 94 of the Pzp Act. 8. In connection with the execution of the order, the Ordering Party does not foresee the obligation to personally perform key tasks of the order. 9. In connection with the execution of the order, the Ordering Party requires that the Contractor and the subcontractor employ persons performing construction works under the supervision of the site manager or branch managers, provided they fall within Art. 22 § 1 of the Act of June 26, 1974 - Labor Code (Journal of Laws of 2019, items 1040, 1043, and 1495). Construction works performed under the supervision of the site manager or branch managers are understood to mean all general construction works such as: installation (electrical, sanitary, telecommunication) and others from various fields resulting from the documentation performed directly by the employees remaining under the supervision of the site manager or branch managers. Details in paragraph III of the SWZ. 10. Source of financing: Project name under the National Reconstruction and Resilience Plan: Investment in the development of cardiology at the St. Wojciech Hospital in Gdańsk - Better Care and Modern Technologies. Component D 'Effectiveness, availability, and quality of the health care system.' Investment D1.1.1. 'Development and modernization of the infrastructure of highly specialized care centers and other medical facilities in the field of cardiology centers qualified for OK II and OK III.' The VAT amount is not an eligible expense within the framework of the above project, and the Ordering Party will finance it from its own funds, partially from the Ordering Party's own funds. 11. The Ordering Party reserves the right to annul the tender procedure for awarding the order if the funds from the subsidy mentioned above, which the Ordering Party intended to allocate for financing the order, are not granted to it. 12. In accordance with the principles of the above-mentioned project, the Ordering Party informs that this procedure (including the range/equipment that is its subject) will be evaluated in terms of compliance with the DNSH principle. This consists of not supporting or conducting economic activities that cause significant harm (serious harm, have a significant negative impact) to any of the environmental objectives. The DNSH principle (Do No Significant Harm) is based on the regulation of the European Parliament and Council (EU) 2020/852 of June 18, 2020, establishing a framework facilitating sustainable investments, known as Taxonomy. Therefore, every venture financed under the National Reconstruction Plan is subject to assessment in terms of 'not doing significant harm'- DNSH.
