The delivery will be carried out as part of project no. KPOD.07.02-IP.10-0093/24, titled: “Improvement of availability and quality of oncological care through the purchase of medical products and devices and modernization of the infrastructure of the Praski Hospital under the patronage of the Transfiguration of the Lord LLC.”, within the funding of D.1.1.1 “Development and modernization of infrastructure of highly specialized care centers and other health care entities.” The subject of the contract is the replacement of the RIS system and the replacement and expansion of PACS servers with the expansion of AI options, full integration with the hospital system, and the ability for remote viewing and description of examinations. Detailed description of the subject of the contract and its implementation can be found in Annex No. 2 to the SWZ – Description of the subject of the contract and in Annex No. 7 to the SWZ – draft contract. Additional information: 1) deliveries must be carried out by an appropriate means of transport and ensured during transport, 2) labeling of delivered products, user instructions, product description, labels (originals) must be drawn up in Polish, 3) the offered subject of the contract must be a brand new product, permitted for trading in the EU, complete and free from technical and legal defects, and must have the manufacturer's name, necessary markings required by regulations (CE), furthermore, both the subject of the contract and its original packaging must contain all other markings resulting from currently applicable regulations, if such are required, 4) all products from specific manufacturers define the minimum quality parameters and performance characteristics that the products must meet in order to meet the requirements set by the Ordering Party and constitute only a quality reference for the subject of the contract, 5) by the provision concerning the minimum requirements of quality parameters, the Ordering Party understands the requirements for goods contained in publicly available sources, catalogs, websites of manufacturers. The examples of manufacturers’ names and products provided by the Ordering Party are only intended to indicate the minimum expectations of the Ordering Party regarding a specific solution. Therefore, the use of manufacturers’ names or products is purely for illustrative purposes. When describing the subject of the contract, the Ordering Party, by indicating the designation of a specific manufacturer (supplier) or a specific product, also allows equivalent products with quality parameters and performance characteristics at least at the level of the indicated product, thereby recognizing every product with the indicated parameters or better. In such cases, the Ordering Party requires the submission of appropriate documents enabling the comparison of equivalent products with the requirements of the Ordering Party. 6) for specified devices, the given parameters are minimum values. Any equipment with better, higher parameters than specified meets the requirements set by the Ordering Party. All devices must comply with all regulations regarding admission to use in Poland and have appropriate documents proving compliance with all necessary standards and guidelines that the aforementioned equipment should meet before being permitted for use, 7) whenever in this SWZ (especially in the description of the subject of the contract) there is mention of materials, devices, software, etc. indicating trademarks, patents, or origins, sources, or a specific process characterizing products or services delivered by a particular contractor, it is assumed that such indications are accompanied by the words “or equivalent.” The designations and proprietary names of products serve only to describe the minimum technical parameters that these products should meet, 8) the equipment constituting the subject of the contract must be brand new, of appropriate quality, operational, free from physical defects, as well as from any legal defects and third-party claims, unused, not for display. By brand new, the Ordering Party understands products made from new elements, without any signs of damage, in the manufacturer's original packaging, e.g. with visible logo, product symbol, 9) delivery to the specified location includes carrying in, assembly, installation (connection), as well as all other costs associated with the execution of the subject of the contract in accordance with the requirements set by the Ordering Party.
