1. The subject of the order: “Computerization of Medical Facilities in the Świętokrzyskie Voivodeship – II”; divided into tasks: a) task number 1 - Integration of the LIS system with the HIS system for storing EDM in the HIS repository along with sending data to the P1 platform; b) task number 2 - Integration of teleradiology with the HIS system.

2. The scope of the subject of the order: The goods offered by the Contractor must: a) comply with Polish standards transferring European standards or standards of other member states of the European Economic Area that transfer those standards, b) meet the requirements specified by the Ordering Party in the Terms of Reference, in particular the conditions specified in the description of the subject of the order.

3. A detailed description of the subject of the order, the description of the Ordering Party's requirements regarding execution and acceptance is specified in: a) annex number 1 – Offer Form, b) annex number 2 – for task number 1 – Description of the subject of the order, c) annex number 2 – for task number 2 – Description of the subject of the order, d) annex number 3 – Draft contract.
