The subject of the order is the delivery of medical equipment for the needs of SPZOZ in Łuków. Detailed description of the subject of the order is included in attachment no. 2 to the specification – Set of technical and functional parameters. The order has been divided into packages: Package no. 1 - Mobile-stationary induction loop, Surgical lamp, Stationary ECG device, Telemedicine data transmission system, Gynecological chair, KTG, Laser therapy device, Ultrasound, Rehabilitation table for massage, Electrotherapy and ultrasound device, Headlight, Package no. 2 - Ultrasound with probe, Gynecological ultrasound, Package no. 3 - OCT eye tomograph. General requirements for the subject of the order: The contractor undertakes to deliver medical equipment that: 1) must be factory new, produced no earlier than 2025, marked and ready to function after installation without any additional purchases and investments, 2) must be unused – i.e. not subject to exhibition and presentation, not refurbished, free from physical and legal defects, 3) must meet the relevant requirements established in applicable legal provisions regarding the admission to use of the aforementioned goods and devices in medical entities, 4) must have a medical device declaration and certificate issued by a notified body (CE mark), and for medical devices class I – must have a manufacturer's declaration of conformity and marking of the product with CE based on the provisions of the Act of April 7, 2022, on medical devices and the Regulation of the European Parliament and Council (EU) 2017/745 of April 5, 2017, 5) must guarantee the safety of patients and medical staff, as well as ensure the required level of provided medical services. 6) at the time of submitting the offer was not planned by the manufacturer to be withdrawn from production or sale. The contractor is obliged to secure its offer with a bid bond in the amount of: package no. 1 - 1000 PLN package no. 2 - 3000 PLN package no. 3 - 1000 PLN The bid bond must be submitted before the deadline for submitting offers. The bond may be submitted in one or more of the following forms: 1) cash; 2) bank guarantees; 3) insurance guarantees; 4) guarantees provided by entities referred to in Art. 6b sec. 5 point 2 of the Act of November 9, 2000, on the establishment of the Polish Agency for Enterprise Development. Required deadline for order fulfillment: package no. 1 - 56 days, package no. 2 - 42 days, package no. 3 - 28 days, from the date of signing the contract.
