The delivery will be carried out under project no. KPOD.07.02-IP.10-0093/24, titled: "Improving the accessibility and quality of oncological care through the purchase of medical products and devices and the modernization of the infrastructure of the Praski Hospital pw. Przemienienia Pańskiego Sp. z o. o.", under funding D.1.1.1 "Development and modernization of infrastructure of highly specialized care centers and other medical entities." The subject of the order is the replacement of the RIS system and the replacement and expansion of PACS servers with the development of AI options, full integration with the hospital system and the possibility of remote viewing and reporting of examinations. Detailed description of the subject of the order and its implementation can be found in Annex No. 2 to the SWZ - Description of the subject of the order and Annex No. 7 to the SWZ - draft contract.
Additional information:
1) Deliveries must be carried out using the appropriate means of transport and secured for transport,
2) Marking of delivered products, user instructions, product description, labels (original) must be prepared in Polish,
3) The offered subject of the order must be brand new, allowed for circulation in the EU, complete and free from technical and legal defects, and must have the manufacturer's name, necessary markings required by regulations (CE), and both the subject of the order and its original packaging must contain all other markings resulting from currently applicable regulations, if such are required,
4) All products from specific manufacturers shall specify the minimum quality parameters and user features that products must meet to meet the requirements set by the Ordering Party and constitute only a quality benchmark for the subject of the order,
5) By the provision regarding minimum quality parameter requirements, the Ordering Party understands the requirements for products contained in publicly available sources, catalogs, manufacturers' websites. The names of manufacturers and products provided by the Ordering Party are merely indicative of the minimum expectations of the Ordering Party regarding a specific solution. Thus, the use of names of manufacturers or products is only illustrative. The Ordering Party, in describing the subject of the order, indicating the designation of a specific manufacturer (supplier) or specific product, also allows equivalent products with quality parameters and user features of at least the level of the indicated product, thus recognizing any product with specified or better parameters. In such a case, the Ordering Party requires the submission of relevant documents enabling the comparison of equivalent products with the Ordering Party's requirements.
6) For specified devices, the given parameters are minimum values. Any equipment with better, higher parameters than those specified meets the requirements set by the Ordering Party. All devices must comply with all regulations regarding admission to use in Poland and possess appropriate documents certifying compliance with all necessary standards and guidelines that the aforementioned equipment should meet before being admitted for use,
7) Whenever this SWZ (specifically in the description of the subject of the order) mentions materials, devices, software, etc. indicating trademarks, patents, or the origin, source, or specific process characterizing products or services provided by a specific contractor, it is assumed that such indications are accompanied by the words "or equivalent." The designations and proprietary names of products serve only to describe the minimum technical parameters that these products should meet,
8) The equipment constituting the subject of the order must be brand new, of appropriate quality, functional, free from physical defects, as well as from any legal defects and claims by third parties, unused, not used for exhibition. By brand new, the Ordering Party means products made of new components, without signs of damage, in original manufacturer packaging, e.g., with visible logo, product symbol,
9) Delivery to the specified location includes the carrying in, assembly, installation (connection), as well as all other costs associated with the execution of the subject of the order in accordance with the requirements set by the Ordering Party.
