Description:
Delivery and installation of medical equipment for the Ophthalmology and Oncology Departments, as well as the Operating Theatre, at the Provincial Specialist Hospital in Słupsk, with detailed specifications provided in the attachments.

1. The subject of the contract is the delivery, including installation, to the Provincial Specialist Hospital named after Janusz Korczak in Słupsk Sp. z o. o. at ul. Hubalczyków 1, of medical equipment in the assortment and quantities specified in Appendix No. 2 to IDW - Assortment and Quantity Form.
2. The subject matter has been described in detail in the attachment to this SWZ and in the template contract - Part II SWZ. The description of the subject of the contract includes a summary of technical parameters, warranty conditions, and training – Part III SWZ. The description of the subject of the contract should be read along with any changes to the content of the specification, which are, for example, the result of provided responses to contractors' inquiries.
