The subject of the order, in accordance with the Functional-Utilitarian Program and the draft contract, which are attached to the SWZ, is to prepare project documentation along with the supply and execution of installation works, namely: 136 photovoltaic installations along with energy storage, 37 photovoltaic installations, and 20 heat pumps, located in the city of Bełchatów in a design and build formula. The subject of the order will be implemented based on the Functional-Utilitarian Program, which is an integral part of the contract. This order is co-financed from the resources of the European Regional Development Fund under the Regional Program European Funds for Łódź 2021 – 2027. Action FELD.02.24 Renewable Energy Sources – ZIT Radomsko-Piotrków Trybunalski-Bełchatów, Priority FELD.02 European funds for green Łódź, implemented under the task: “Purchase and installation of renewable energy systems in residential buildings in the city of Bełchatów.” The contracting authority anticipates the use of the procedure referred to in Article 139 paragraph 1 of the PZP Act, that is, the possibility of applying the so-called reversed order of assessment of offers. In this case, the contracting authority will first conduct an examination and evaluation of offers and then qualify the contractor, whose offer received the highest evaluation, in terms of the lack of grounds for exclusion and meeting the participation conditions in the proceedings. The subject of the order has not been divided into parts. The contracting authority does not allow for partial offers. Detailed justification is indicated in the procurement documents. 5. The contracting authority does not allow for variant offers or offers in the form of electronic catalogs. 6. The contracting authority does not anticipate awarding contracts referred to in Article 214 paragraph 1 point 8 of the PZP Act. 7. A detailed description and method of execution of the order are contained in the Functional-Utilitarian Program and the draft contract, which are attachments to the SWZ. 8. Warranty and liability: the required warranty period for the completed subject of the contract: minimum 60 months, counting from the date of final acceptance; - required liability period for the completed subject of the contract: minimum 60 months, counting from the date of final acceptance. NOTE: evaluation criterion for offers. Deadline for execution of the order: 4 months. 2. The installations with the heat pump must be completed and launched by October 15, 2026. 3. Detailed issues related to the contract execution deadline are regulated in §4 of the draft contract – Attachment to the SWZ. STATEMENTS AND DOCUMENTS THAT CONTRACTORS ARE OBLIGED TO PROVIDE TO DEMONSTRATE THE LACK OF GROUNDS FOR EXCLUSION AND CONFIRM MEETING PARTICIPATION CONDITIONS 1. The contractor is obliged to include in the offer the current on the day of submission of offers: 1) a completed and signed offer form prepared based on the template specified in the Attachment to the SWZ; 2) Completed and signed statement of the contractor regarding the grounds for exclusion from art. 5k of Regulation No. 2022/576 of April 8, 2022, amending Council Regulation (EU) No. 833/2014 concerning restrictive measures in connection with actions by Russia destabilizing the situation in Ukraine and Article 7 paragraph 1 of the Act on special solutions to counteract supporting aggression against Ukraine and serving to protect national security – according to the template specified in the Attachment to the SWZ; 3) Completed and signed statement of the entity providing resources concerning grounds for exclusion from art. 5k of Regulation No. 2022/576 of April 8, 2022, amending Council Regulation (EU) No. 833/2014 concerning restrictive measures in connection with actions by Russia destabilizing the situation in Ukraine and Article 7 paragraph 1 of the Act on special solutions to counteract supporting aggression against Ukraine and serving to protect national security – according to the template specified in the Attachment to the SWZ – if applicable; 4) Completed and signed commitment of the entity providing resources to make the necessary resources available to the contractor for the purposes of execution of the given order (legal basis: Article 118 paragraph 3 of the PZP Act) – according to the Attachment to the SWZ; or another means of proof confirming that the contractor executing the order will have the necessary resources of these entities – if applicable; 5) Completed and signed statement of contractors jointly applying for the award of the contract, submitted based on Article 117 paragraph 4 of the Act of September 11, 2019 according to the template specified in the Attachment to the SWZ – if applicable. 6) Other documents necessary for the conduct of the proceedings: a) documents proving the authorization of persons to represent the contractor, in particular an excerpt or information from the National Court Register, the Central Register and Information on Economic Activity or another relevant register. b) a power of attorney to represent in the proceedings for the award of the public contract or representation in the proceedings and conclusion of the contract regarding the public procurement of contractors applying jointly in case of applying jointly for the award of this contract; 2.
