The subject of the contract is the supply of reagents along with the lease of a set of devices for conducting hematology tests and kits for RNA isolation compatible with the QuicGeneAuto12S device for a period of 12 and 36 months, divided into 2 parts. Detailed description of the subject of the order constitutes Attachment No. 2 to the SWZ - Product and price form, Attachment No. 2.1 to the SWZ - Form of required technical parameters and assessed (applies to part 1) Minimum shelf life of the goods covered by the subject of the order is at least 12 months, counting from the delivery date to the Ordering Party. Deliveries of products with a shorter shelf life may be permitted in exceptional situations and each time the consent for them must be expressed by an authorized representative of the Ordering Party. The terms of execution of the Subject of the Order have also been included in the draft provisions of the public procurement contract, which will be introduced into the content of this contract, constituting Attachment No. 4 to the SWZ (applies to Part 2) and Attachment No. 4.1. to the SWZ. (applies to Part 1) OPTION RIGHT: The Ordering Party, acting on the basis of the provision of Article 441 of the Act, within the option right may commission the Contractor to perform additional deliveries, in accordance with the subject of the basic order, referred to in the description of the subject of the order, constituting Attachment No. 2 to the SWZ Product and price form. The Ordering Party will exercise the option right referred to in the above point by submitting declarations in this regard. The Ordering Party is entitled, at its own choice, to submit a declaration regarding the order granted under the option right several times or once, until the maximum value specified in § 2 sec. 5 of Attachment No. 4 to the SWZ Draft Provisions of the Contract (applies to Part 2) and § 2 sec. 5 of Attachment No. 4.1. to the SWZ Draft Provisions of the Contract (applies to Part 1) is utilized. The maximum value covered by the option right shall not exceed 20% of the Contractor's compensation resulting from the concluded contract.
