1. The subject of the contract is the expansion of the Ordering Party's IT systems along with the digitization of medical documentation and integration with the Intelligent Services Platform at the "Poddębickie Centrum Zdrowia" Sp. z o.o. in Poddębice, including in particular the execution of the following tasks:
I. Expansion of the HIS system - Radiation dose register;
II. Expansion of the HIS system - integration with Teleradiology;
III. Expansion of the HIS system - TNM;
IV. Integration of new EDM documents with P1;
V. Granting licenses for server operating systems;
VI. Granting licenses for access software for the authentication system;
VII. Granting licenses for virtualization system software;
VIII. Delivery of matrices;
IX. Delivery of virtualization servers;
X. Delivery of SAN switches;
XI. Technical support service for servers;
XII. Expansion of the RIS/PACS system;
XIII. Implementation of the PACS/RIS interface - PUI.
2. A detailed description of the contract is included in Annex No. 1 to the SWZ - Offer Form, Annex No. 2 to the SWZ - Description of the subject of the contract and Annex No. 5 to the SWZ - Draft provisions of the contract for public procurement, which will be included in the content of this contract (contract template).
3. The contract is being carried out as part of the project titled: “Increasing the level of digitization of the Poddębickie Centrum Zdrowia” co-financed under the National Reconstruction and Resilience Plan, Component D, Investment D1.1.2 “Accelerating the processes of digital transformation of health care through further development of digital services in health care”, recruitment No. KPOD.07.03-IP.10-001/25, based on contract No. KPOD.07.03-IP.10-0044/25/KPO/2267/2025/296.
4. Contractors who have any of the circumstances referred to in Article 108(1) of the Act on Public Procurement and Article 109(1)(4) of the Act on Public Procurement are excluded from the procurement procedure. The languages in which procurement documents are officially available: Polish.
