1. The subject of the order is the delivery and implementation of technical-system infrastructure, equipment, and software, along with the provision of services under the project entitled 'Digitization of Medical Services and Increasing the Level of Cybersecurity at the Jan Pawel II Provincial Hospital in Bełchatów'. The order is part of Investment D1.1.2 Accelerating the processes of digital transformation in healthcare through further development of digital services in healthcare, funded by the European Recovery and Resilience Facility (National Recovery and Resilience Plan - KPO), based on Agreement No. KPOD. 07.03-IP.10 0281/25/KPO/662/2025/588 for support from the developmental plan for the undertaking mentioned in paragraph 1, entered into between the State Treasury - Minister of Health and the Jan Pawel II Provincial Hospital in Bełchatów. 2. Code and name according to the Common Procurement Vocabulary (CPV): The contracting authority described the subject of the order using, among other things, names and codes specified in the Common Procurement Vocabulary. Depending on the description of a given part of the order, the contracting authority indicated the appropriate CPV codes assigning them to individual tasks, in accordance with Chapter XXIII of the SWZ. At the same time, taking into account the rules for publishing announcements for orders above the EU thresholds in the Official Journal of the European Union, the contracting authority additionally indicates the code and name for the entire procedure, i.e., 48000000-8 Software packages and information systems. 3. Description, size, and scope of the subject of the order: 1) The contracting authority divided the subject of the order into 5 parts (Tasks No. 1–5), in accordance with the description provided in Chapter XXIII of the SWZ. 2) A detailed description of the subject of the order can be found in Attachment No. 7 to the SWZ, separately for each task. 3) Other requirements regarding the execution of the order and the principles of cooperation between the contracting authority and the contractor are specified in the draft provisions of the public procurement agreement, constituting Attachment No. 3 to the SWZ.
