The subject of the order is the purchase, delivery, and implementation of an IT solution that constitutes the expansion and modernization of the Hospital's domain systems, enabling the integration and exchange of data from the RIS/PACS systems with the Intelligent Services Platform (PUI) developed by the e-Health Center, in terms of utilizing artificial intelligence (AI) mechanisms that support the diagnostic-therapeutic process, along with providing a warranty covering author supervision and service for a period of 36 months in accordance with the description and requirements specified in attachment No. 2 (SOPZ) and 3 (contract template) to the SWZ. 2. A detailed description of the subject of the order and the requirements of the Contracting Authority regarding the subject of the order are specified in attachment No. 3 to the SWZ (draft contract) and attachment No. 2 to the SWZ (description of the subject of the order). Documents comprising the offer: a) Offer Form (attachment No. 1 to the SWZ), made available by the Contracting Authority on the e-Procurement Platform and posted in the overview of the proceedings in the “Basic Information” tab, signed with a qualified electronic signature by persons duly authorized to represent the Contractor, b) documents and declarations confirming that the Contractor meets the conditions for participation in the Proceedings and the lack of grounds for exclusion mentioned in the Specification, c) in the case of Contractors acting through an attorney – Power of Attorney, d) in the case of Contractors jointly applying for the order – a document stating the establishment by the Contractors jointly applying for the order of a proxy to represent them in the proceedings for the award of the contract or representing in the proceedings and concluding a contract for public procurement, e) confirmation of the submission of the bid bond. The Contracting Authority, according to art. 139 sec. 1 of the Pzp Act informs that first it will carry out the examination and evaluation of offers, and then it will qualify the Contractor whose offer has been ranked highest, regarding the absence of grounds for exclusion and meeting the conditions for participation in the proceedings.
