The delivery will be carried out as part of project no. KPOD.07.02-IP.10-0093/24, entitled: "Improving access to and quality of oncology care through the purchase of medical products and devices, and modernization of the infrastructure of the Praski Hospital under the patronage of the Transfiguration of Our Lord Ltd." with funding D.1.1.1 "Development and modernization of the infrastructure of high-specialty care centers and other medical entities."
The subject of the order is the replacement of the RIS system and the replacement and expansion of PACS servers with the expansion of AI options, complete integration with the hospital system, and the ability for remote review and description of examinations.
The detailed description of the subject of the order and its implementation can be found in Annex No. 2 to the SWZ - Description of the subject of the order and Annex No. 7 to the SWZ - draft contract.
Additional information:
1) deliveries must be made using appropriate means of transport and secured during transport,
2) labeling of delivered products, user instructions, product description, labels (original) must be prepared in Polish,
3) the offered subject of the order must be a brand new product, permitted for sale in the EU, complete and free from technical and legal defects, and must also have the manufacturer's name, necessary markings required by regulations (CE). Additionally, both the subject of the order and its original packaging must contain any other markings resulting from currently applicable regulations, if such are required,
4) all products from specific manufacturers define the minimum quality parameters and functional characteristics that must be met for the products to meet the requirements set by the Ordering Party and constitute only a quality standard for the subject of the order,
5) by the provision regarding minimum quality parameter requirements, the Ordering Party understands the requirements for goods contained in generally available sources, catalogs, and manufacturers' websites. The sample manufacturer and product names provided by the Ordering Party serve solely to indicate the minimum expectations of the Ordering Party regarding a specific solution. Thus, the use of the names of manufacturers or products is only illustrative. When describing the subject of the order, the Ordering Party indicates the designation of a specific manufacturer (supplier) or specific product while simultaneously allowing equivalent products with quality parameters and functional characteristics at least at the level of the specified product, thus recognizing every product with the specified or better parameters. In such situations, the Ordering Party requires the submission of appropriate documents enabling the comparison of equivalent products with the Ordering Party's requirements.
6) for specified devices, the given parameters are minimum values. Any equipment with parameters better or higher than the specified meets the requirements set by the Ordering Party. All devices must comply with all regulations regarding admission to use in Poland and possess appropriate documents proving compliance with all necessary standards and guidelines that the aforementioned equipment should meet before being admitted for use,
7) whenever this SWZ refers to materials, devices, computer programs, etc. with trademarks, patents, or origins, sources, or specific processes characterizing the products or services delivered by a specific contractor, it is assumed that such indications are accompanied by the words “or equivalent”. The designations and brand names of products serve solely to describe the minimum technical parameters that these products should meet,
8) the equipment constituting the subject of the order must be brand new, of suitable quality, functional, free from physical defects, as well as from any legal defects and claims of third parties, unused and not exhibited. By brand new, the Ordering Party understands products made from new components, without any signs of damage, in the original manufacturer's packaging, e.g., with a visible logo, product symbol,
9) delivery to the specified location includes carrying in, assembly, installation (connection), and all other costs associated with the execution of the subject of the order in accordance with the requirements set by the Ordering Party.
