The delivery will be carried out under project no. KPOD.07.02-IP.10-0093/24, entitled: "Improving access and quality of oncological care through the purchase of medical products and devices and modernization of the infrastructure of the Praski Hospital named after the Transfiguration of the Lord Sp. z o. o.", under funding D.1.1.1 "Development and modernization of the infrastructure of specialized care centers and other healthcare entities."
The subject of the contract is the replacement of the RIS system and the replacement and expansion of the PACS servers with expanded AI options, full integration with the hospital system, and the capability for remote review and reporting of examinations.
A detailed description of the subject of the contract and its execution is included in Annex No. 2 to the SWZ - Description of the subject of the contract and in Annex No. 7 to the SWZ - draft contract.
Additional information:
1) deliveries must be realized using appropriate means of transport and secured during transport,
2) labeling of the delivered products, user instructions, product descriptions, labels (original) must be prepared in Polish,
3) the offered subject of the contract must be a brand new product, allowed for trading in the EU, complete and free from technical and legal defects, and must contain the manufacturer's name and all necessary markings required by regulations (CE), furthermore both the subject of the contract and its original packaging must contain any other markings required by currently applicable regulations, wherever such requirements exist,
4) all products from specific manufacturers specify the minimum quality parameters and functional characteristics that the products must meet in order to meet the requirements set by the Ordering Party and constitute only a quality pattern of the subject of the contract,
5) by the provision concerning minimum quality parameter requirements, the Ordering Party understands the requirements of goods contained in publicly available sources, catalogs, producers' websites. The examples of manufacturers and products given by the Ordering Party aim solely to indicate the minimum expectations of the Ordering Party regarding a specific solution. Thus, the use of names of manufacturers or products is only exemplary. In describing the subject of the contract with a reference to a particular manufacturer (supplier) or specific product, the Ordering Party also accepts equivalent products with quality parameters and functional characteristics, at least at the level of the parameters of the indicated product, thereby recognizing any product with the indicated parameters or better. In such cases, the Ordering Party requires the submission of appropriate documents enabling the comparison of equivalent products with the requirements of the Ordering Party.
6) for the specified devices, the provided parameters are minimum values. Any equipment with parameters better than those specified meets the requirements set by the Ordering Party. All devices must meet all regulations regarding admission to use in Poland and possess appropriate documents proving compliance with all necessary standards and guidelines that the aforementioned equipment must meet before it is allowed for use,
7) whenever in this SWZ (especially in the description of the subject of the contract) there are references to materials, devices, computer programs, etc. with the indication of trademarks, patents, or origin, source or specific process characterizing products or services offered by a specific contractor, it is assumed that such references are accompanied by the words "or equivalent." The designations and proper names of products serve solely to describe the minimum technical parameters that these products should meet,
8) the equipment being the subject of the contract must be brand new, of proper quality, functional, free from physical defects, as well as from any legal defects and claims of third parties, unused, not exhibited. By brand new, the Ordering Party understands products made from new components, without signs of damage, in the original packaging of the manufacturer, e.g. with visible logo, product symbol,
9) delivery to the specified location includes carrying in, assembly, installation (connection) and all other costs related to the execution of the subject of the contract in accordance with the requirements set by the Ordering Party.
