1. The subject of the order is the purchase of medical equipment for imaging diagnostics as part of the investment "Modernization and development of the oncology infrastructure at the Municipal Hospital in Siemianowice Śląskie Sp. z o. o.", in accordance with annex no. 1 to the specification – Detailed description of the object of the order. The quality and service warranty conditions are included in annex no. 1.1 to the specification.
2. The order also includes:
3. transport of the device (hereinafter in SWZ the equipment/device) to the destination,
4. installation, commissioning,
5. connecting the supplied equipment to the system of PACS/RIS owned by the Ordering Party and configuring it for proper operation with this system,
6. staff training and transfer of medical equipment for use along with all necessary approvals, certificates,
7. collaboration with the Ordering Party in preparing documentation to obtain all necessary permits for the use of the subject of the order, including the permit from the State Provincial Sanitary Inspector in Katowice.
8. The subject of the order is divided into 3 parts/packages, according to the description of the subject of the order constituting annex no. 1 to the SWZ
1. Intraoperative X-ray machine
2. Mobile bedside X-ray machine – digital
3. Mammograph.
The task will be co-financed as part of the National Reconstruction and Resilience Plan – component D "Efficiency, availability, and quality of the healthcare system", investment D1.1.1 Development and modernization of infrastructure of highly specialized care centers and other healthcare entities.
